Myelinated Fiber Blockade

Myelinated fiber blockade is the interruption of action potential conduction along axons wrapped in myelin, a process that matters in pharmacology because it can reduce or eliminate nerve signaling. Pharmacological blockers, especially local anesthetics, inhibit voltage-gated sodium channels at the nodes of Ranvier, preventing the depolarization required for saltatory conduction and producing a reversible conduction block when sufficient drug reaches the nerve. This principle supports regional and local anesthesia, where sensory transmission is suppressed, and helps researchers evaluate how drug concentration, fiber diameter, and myelin structure influence nerve susceptibility and recovery.

The site of chemical communication between a motor neuron and a muscle fiber is called the neuromuscular junction (NMJ). Deep Neuromuscular Blockade Leads to a Larger Intraabdominal Volume During Laparoscopy

This article presents a method for measuring the working abdominal volume during laproscopic myomectomy using the surgical grasper, and applies this technique to obtain pilot data on whether deep neuromuscular blockade (NMB) can enable the use of lower insufflation pressure. Reduced insufflation pressure during laproscopic surgery has been shown to reduce post-operative pain, and thus the use of NMB during surgery may enable improved patient outcomes.
