Physiology of the Circulatory System

La fisiologia è una branca della biologia che descrive il funzionamento e l'adattamento degli organismi ai diversi ambienti. Nell'uomo, una misura della funzione del sistema circolatorio può essere ottenuta misurando la pressione sanguigna. Il termine pressione sanguigna descrive la pressione laterale prodotta dal sangue sulle pareti dei vasi. 
Una delle applicazioni più importanti della misurazione della pressione sanguigna è il controllo dell'aumento della pressione sanguigna, una condizione chiamata ipertensione. Un adulto su tre negli Stati Uniti soffre di ipertensione ed è una delle principali cause di malattie cardiovascolari. L'attrezzatura necessaria per una misurazione indiretta tradizionale della pressione sanguigna comprende uno stetoscopio e uno sfigmomanometro. Lo sfigmomanometro è costituito da un bracciale per la pressione sanguigna contenente una vescica estensibile e da un bulbo di gomma con una valvola regolabile che, quando è chiuso, aiuta nel gonfiaggio del bracciale e, quando è aperto, rilascia la pressione accumulata. Consiste anche in un tubo che collega il bracciale al bulbo e al manometro che visualizza la pressione del bracciale in millimetri di mercurio. Per registrare la lettura della pressione sanguigna, l'esaminatore avvolge il bracciale attorno all'arteria brachiale, posiziona uno stetoscopio su questa arteria, gonfia il bracciale al di sopra della pressione sistolica prevista e quindi lo sgonfia mentre ausculta e osserva contemporaneamente il manometro. Inizialmente, quando il bracciale è completamente gonfiato, l'arteria viene schiacciata e il flusso sanguigno viene interrotto. Pertanto, non c'è suono durante l'auscultazione. Al momento dello sgonfiaggio, la prima comparsa dei suoni di Korotkoff indica la pressione sistolica, che è udibile a causa del flusso turbolento nell'arteria parzialmente schiacciata. Un ulteriore sgonfiaggio provoca una continua diminuzione della pressione della cuffia e i suoni di Korotkoff rimangono udibili per tutto il tempo, fino al punto in cui la pressione della cuffia è inferiore alla pressione arteriosa minima. Questa lettura denota la pressione diastolica. La frazione di sistolica su diastolica viene registrata come lettura finale della pressione sanguigna. 
Un altro indicatore importante della fisiologia umana è la frequenza respiratoria, il numero di respiri al minuto. L'aria che respiriamo viaggia attraverso la trachea, nei polmoni attraverso i bronchi. All'interno dei polmoni passa attraverso i bronchioli per entrare infine nelle sacche d'aria specializzate chiamate alveoli. Gli alveoli sono circondati da capillari sanguigni che permettono la diffusione dell'ossigeno inalato nel nostro flusso sanguigno e facilitano l'escrezione di anidride carbonica, mantenendo così l'omeostasi del nostro sistema. Per i mammiferi come gli esseri umani, il mantenimento dell'omeostasi include il mantenimento di una temperatura corporea relativamente costante, indipendente dalla temperatura ambientale circostante. Questa proprietà ci rende endotermi. Pertanto, il nostro tasso metabolico e la frequenza respiratoria rimangono relativamente costanti. Al contrario, quasi tutti i pesci, compresi i pesci rossi, hanno una temperatura corporea interna che quasi corrisponde alla temperatura dell'ambiente che li circonda. Tali organismi sono chiamati ectotermi. Poiché non mantengono attivamente una temperatura interna costante, la loro frequenza metabolica e respiratoria cambia per compensare la variazione della temperatura ambientale circostante. La frequenza respiratoria nei pesci può essere misurata contando i battiti branchiali, l'ascesa e la discesa dell'opercolo o della copertura branchiale, al minuto. C'è un altro fattore che influenza il tasso di respirazione degli organismi acquatici. Si noti che un pesce respira l'ossigeno disciolto presente nel suo ambiente. E la quantità di ossigeno disciolto disponibile nell'acqua dipende dalla temperatura dell'acqua. Diminuisce con l'aumentare della temperatura che, a sua volta, influisce anche sulla frequenza respiratoria. 
In questo laboratorio esaminerai il sistema circolatorio umano misurando la pressione sanguigna in diverse condizioni, e poi studierai gli effetti della temperatura sulla respirazione usando i pesci.

<h4>Omeostasi</h4>
Le condizioni nell'ambiente esterno di un organismo possono cambiare rapidamente e drasticamente. Per sopravvivere, gli organismi devono mantenere un ambiente interno abbastanza costante, che comporta una regolazione continua della temperatura, del pH e di altri fattori. Questo stato di equilibrio è noto come omeostasi, che descrive i processi attraverso i quali gli organismi mantengono le loro condizioni interne ottimali. <h4>Sistemi di organi nell'uomo</h4>
Uno o più tipi di cellule con funzioni di supporto formano tessuti, che a loro volta costituiscono organi con funzioni corporee specializzate. I sistemi di organi sono costituiti da due o più organi che lavorano insieme per fornire una funzione comune. Il sistema fisiologico dei vertebrati comprende 11 principali sistemi di organi. Sebbene tutti i sistemi di organi siano interconnessi, funzionano in qualche modo indipendentemente l'uno dall'altro.
Il corpo è coperto dal sistema tegumentario, che consiste di pelle, capelli, unghie, recettori sensoriali e varie ghiandole. Il sistema respiratorio controlla lo scambio di gas per fornire ossigeno al corpo mentre rimuove l'anidride carbonica nei polmoni dopo il passaggio dell'aria attraverso la cavità nasale, la faringe, la laringe, la trachea e i bronchi. L'apparato digerente elabora e scompone il cibo che viene assunto attraverso la cavità orale e l'esofago, e poi spostato attraverso lo stomaco, l'intestino tenue e crasso prima dell'escrezione attraverso il retto e l'ano. Infine, il sistema riproduttivo innesca le cellule riproduttive degli organismi. Nei maschi, i testicoli e il pene costituiscono il sistema riproduttivo, mentre nelle femmine l'utero, le ovaie e la vagina costituiscono il sistema riproduttivo.
<h4>Fisiologia del sistema circolatorio</h4>
La fisiologia degli organismi unicellulari e degli animali multicellulari basali, come le spugne, è spesso semplice. Ad esempio, le piccole dimensioni e l'ampio rapporto superficie/volume dei microrganismi consentono loro di ottenere la regolazione per diffusione attraverso la membrana cellulare. Allo stesso modo, l'acqua di mare circola attraverso i pori delle spugne, trasportando sostanze nutritive e prodotti di scarto da e verso le sue cellule. Gli animali più complessi hanno sviluppato sistemi circolatori per spostare il sangue in tutto il corpo per trasportare nutrienti, prodotti di scarto, ormoni e altre molecole, mentre i sistemi respiratori consentono lo scambio di gas tra il sistema circolatorio e l'ambiente esterno. 
Il sistema circolatorio negli animali può essere aperto o chiuso. I sistemi circolatori aperti sono tipicamente presenti in molti invertebrati e sono costituiti da uno o più cuori semplici, una rete di vasi e spazi interconnessi che immergono direttamente gli organi interni in un fluido che consente lo scambio di materiali. I vertebrati hanno sistemi circolatori chiusi, in cui il sangue è confinato all'interno di un sistema di vasi chiusi che si ramifica ampiamente nei tessuti per garantire lo scambio di materiale. Questo sistema di vasi chiusi è collegato a un cuore con vene che trasportano il sangue dai tessuti verso il cuore e le arterie che trasportano il sangue dal cuore al resto del corpo. I cuori a quattro camere, come negli esseri umani, sono associati a due anelli di vasi. Negli esseri umani, il sangue impoverito di ossigeno proveniente dagli organi entra nel cuore attraverso l'atrio destro, che si contrae per spingere il sangue verso il ventricolo destro, che a sua volta invia il sangue ai polmoni. A seguito dello scambio di gas nei polmoni, il sangue ricco di ossigeno ritorna nell'atrio sinistro e viene successivamente spinto nel ventricolo sinistro. Quest'ultima camera è più muscolosa delle altre e con una forte contrazione, è in grado di pompare il sangue al resto del corpo.
I sistemi circolatori chiusi consentono una rapida circolazione del sangue, che a sua volta consente un trasporto rapido ed efficiente delle sostanze in tutto il corpo, nonché una pressione sanguigna più elevata rispetto ai sistemi aperti. La pressione sanguigna è generata dalla contrazione dei ventricoli cardiaci quando il sangue viene forzato nelle arterie. Man mano che i ventricoli del cuore si rilassano, la pressione sanguigna diminuisce.
<h4>Pressione sanguigna</h4>
Nell'uomo, il funzionamento del sistema circolatorio può essere valutato misurando la pressione sanguigna e la frequenza cardiaca corrispondente in un individuo. La pressione sanguigna è misurata in millimetri di mercurio (mmHg), che è l'altezza in millimetri a cui il mercurio in una colonna viene sollevato a causa della pressione esercitata su di esso. La frequenza cardiaca viene misurata in battiti al minuto. A causa dei movimenti di contrazione e rilassamento del cuore, le letture della pressione sanguigna sono costituite da due numeri: sistolica e diastolica. La pressione sistolica viene misurata durante le contrazioni dei ventricoli e la pressione diastolica è la pressione minima nelle arterie durante il riposo tra le contrazioni. Generalmente, le pressioni sistoliche di 90-120 mmHg e le pressioni diastoliche di 60-80 mmHg sono considerate sane. Quando si tratta di frequenza cardiaca, 60-100 battiti al minuto sono considerati salutari per gli adulti. Gli atleti hanno generalmente una frequenza cardiaca più bassa poiché gli esercizi cardiovascolari aumentano la frequenza cardiaca e condizionano il cuore a pompare in modo più efficiente, il che alla fine riduce la frequenza cardiaca a riposo1.
Livelli elevati di pressione sanguigna per periodi prolungati o ipertensione possono danneggiare i vasi sanguigni ed è stato collegato a infarto e ictus2. I ricercatori hanno scoperto che gli effetti cardiovascolari della pressione sistolica e diastolica sono diversi, in modo tale che i tassi di eventi cardiovascolari sono fortemente associati alla pressione sistolica. Pertanto, il numero di pazienti con ipertensione sistolica che soffrono di eventi cardiovascolari è superiore al numero di pazienti con ipertensione diastolica3. Fattori genetici, così come lo stile di vita e i fattori ambientali, possono causare ipertensione e malattie cardiovascolari. Ad esempio, il consumo di elevate quantità di sale provoca la ritenzione di acqua in eccesso nel corpo, aumentando la pressione sanguigna e affaticando i vasi sanguigni. Qualsiasi insulto ai vasi sanguigni li rende inclini a lesioni, il che fornisce superfici per l'accumulo di placca, irrigidendo infine i vasi sanguigni e riducendo l'efficienza del flusso sanguigno.
Misurazioni della pressione sanguigna
Gli sfigmomanometri sono utilizzati per misurare la pressione sanguigna. Sono composti da un bracciale gonfiabile collegato a una pompa (manuale o automatica) e da un manometro. Il modo più efficiente per utilizzare uno sfigmomanometro è sull'arteria brachiale sulla parte superiore del braccio, che è a livello del cuore. Gli sfigmomanometri vengono utilizzati in combinazione con uno stetoscopio, che è un dispositivo medico acustico utilizzato per ascoltare i suoni interni tramite un disco metallico o un risonatore. Lo stetoscopio viene tenuto appena sotto lo sfigmomanometro, appena sopra l'interno del gomito dell'individuo per misurare i suoni della pressione sanguigna sistolica e diastolica. Il bracciale è gonfiato a 200 mmHg interrompendo il flusso sanguigno pizzicando i vasi sanguigni ed è una quantità sicura di pressione da applicare al braccio. Quando il bracciale si sgonfia, i vasi sanguigni iniziano ad aprirsi e il sangue può fluire di nuovo. La pressione arteriosa sistolica è indicata dal primo rumore udito e la pressione diastolica è determinata dall'ultimo rumore udito. Questi rumori sono chiamati suoni di Korotkoff, che sono il suono del sangue che viene spinto con forza attraverso i vasi dal cuore4.
<h4>Apparato respiratorio</h4>
Il sistema circolatorio lavora a stretto contatto con il sistema respiratorio per fornire ossigeno ai tessuti rimuovendo l'anidride carbonica. Diversi organismi hanno sviluppato strutture respiratorie distinte per lo scambio di gas. Ad esempio, molti animali acquatici scambiano gas attraverso le branchie. I movimenti branchiali sono facilmente osservabili e possono essere utilizzati per calcolare il tasso di respirazione degli organismi acquatici contando il numero di volte in cui l'organismo muove la copertura branchiale o l'opercolo. La frequenza respiratoria può cambiare con la temperatura perché le molecole di ossigeno vengono trasportate a velocità diverse a seconda di quanto sia caldo o freddo un ambiente. In un ambiente acquatico, la quantità di ossigeno disciolto disponibile nell'acqua diminuisce con l'aumentare della temperatura. Il fumo è la principale causa di cancro ai polmoni, responsabile dell'80-90% dei decessi per cancro ai polmoni. The circulatory system moves blood, oxygen, nutrients, and wastes through the body to support homeostasis. Homeostasis is the stable internal balance that organisms must maintain even when the outside environment changes. Physiology is the study of how body systems work, and the circulatory system is one of the key systems that helps keep internal conditions steady.

Body systems are built from cells, tissues, organs, and organ systems. Humans have 11 major organ systems, and they work together even though each system has its own main job. The integumentary system protects the body and helps control temperature. The skeletal system supports organs and gives muscles a place to attach. The muscular system moves the body and can also move substances through hollow organs. The nervous system controls responses to stimuli. The endocrine system uses hormones to regulate growth, metabolism, and reproduction. The respiratory system exchanges gases in the lungs. The digestive system breaks down food and absorbs nutrients. The urinary system removes nitrogenous wastes and extra water. The lymphatic system helps immune defense. The reproductive system produces reproductive cells.

In simple organisms, transport can happen by diffusion because of their small size and large surface area to volume ratio. Sponges also rely on water flow through their pores to carry nutrients and wastes. More complex animals need circulatory systems to move materials farther and faster. These systems transport nutrients, waste products, hormones, and other molecules, while the respiratory system supports gas exchange with the outside environment.

Animal circulatory systems can be open or closed. Open circulatory systems are common in many invertebrates. They use one or more simple hearts, a vessel network, and open spaces that bathe organs in fluid for exchange. Vertebrates have closed circulatory systems, where blood stays inside vessels that branch through tissues. In this system, veins carry blood toward the heart, and arteries carry blood away from the heart.

Humans have a four-chambered heart with two circulation loops. Oxygen-poor blood from the body enters the right atrium, moves to the right ventricle, and is sent to the lungs. After gas exchange, oxygen-rich blood returns to the left atrium and then moves to the left ventricle. The left ventricle has a thicker muscle wall because it must pump blood to the rest of the body. Closed circulatory systems move blood quickly and efficiently and produce higher blood pressure than open systems.

Blood pressure and heart rate are used to measure how well the circulatory system is working. Blood pressure is measured in millimeters of mercury, or mmHg. Heart rate is measured in beats per minute. Blood pressure has two numbers. Systolic pressure is the pressure during ventricular contraction. Diastolic pressure is the lowest arterial pressure between contractions. Healthy adult values are usually about 90-120 mmHg for systolic pressure and 60-80 mmHg for diastolic pressure. A healthy adult heart rate is usually 60-100 beats per minute, and athletes often have lower resting heart rates because exercise makes the heart pump more efficiently.

High blood pressure, or hypertension, can damage blood vessels and is linked to heart attack and stroke. The transcript notes that systolic pressure is strongly associated with cardiovascular events, and systolic hypertension is more closely linked to those events than diastolic hypertension. Hypertension can result from genetic, lifestyle, and environmental factors. For example, eating a lot of salt can cause the body to retain extra water. That raises blood pressure and strains blood vessels. Injured vessel walls can also encourage plaque buildup, which can stiffen vessels and reduce blood flow.

Blood pressure is often measured with a sphygmomanometer and a stethoscope. A sphygmomanometer uses an inflatable cuff, a pump, and a pressure gauge. It is usually placed on the brachial artery in the upper arm, level with the heart. The cuff is inflated to 200 mmHg, which briefly stops blood flow and is safe for the arm. As the cuff deflates, blood starts flowing again. The first sound heard is the systolic pressure, and the last sound heard is the diastolic pressure. These sounds are called Korotkoff sounds, and they come from blood being forced through the vessels.

The circulatory system also works closely with the respiratory system to supply oxygen and remove carbon dioxide. Many aquatic animals use gills for gas exchange, and their respiration rate can be estimated by counting operculum movements. Respiration rate can change with temperature because oxygen moves differently in warm or cool environments. In water, dissolved oxygen decreases as temperature rises, which affects aquatic organisms that rely on diffusion. Terrestrial animals use internal structures such as lungs. In humans, inhalation happens when the diaphragm contracts and pulls air in, and exhalation happens when the diaphragm relaxes. Smoking is the leading cause of lung cancer and accounts for 80-90% of lung cancer deaths. The transcript also notes that more than 120,000 Americans die each year from smoking-associated lung cancer, showing how lifestyle choices can affect both circulatory and respiratory health.

EXPLANATION: Discussions of DNA in earlier grades have been limited to discussions of genes and the role of genes in the appearance and activities of an organism. Biology 1 represents a student's introduction to a molecular model of a DNA as well as the organization of DNA into genes and genes subsequently into chromosomes. Students should address interactions between genes in proteins which regulate both the shape and reproduction of DNA molecules. It is important to note that not all segments of DNA are transcribed for proteins. Portions of DNA are involved in regulating the expression of genes or affect the structure of the chromosomes (replication origin, centromeres, and telomeres).
