Anal Sphincter Tone

Anal sphincter tone is the baseline contractile state of the muscles surrounding the anal canal, helping maintain continence between bowel movements. It reflects coordinated activity of the involuntary internal anal sphincter, composed of smooth muscle, and the voluntary external anal sphincter, composed of skeletal muscle; rectal distension triggers reflex relaxation of the internal sphincter, while conscious contraction of the external sphincter can delay defecation. Clinicians assess tone through digital rectal examination and, when needed, anorectal manometry to measure resting and squeeze pressures. Abnormal tone may contribute to fecal incontinence or obstructed defecation, informing diagnosis, treatment planning, and evaluation of pelvic floor disorders.

We have developed a device (Twister) to study the regulation of tonic muscle activity during active postural maintenance. Twister measures torsional resistance and muscular responses in standing subjects during twisting of the body axis. The device can be flexibly configured to study various aspects of tonic control across the neck, trunk, and/or hips.
