Liver Extraction Ratio

Liver extraction ratio is the fraction of a drug removed from the blood during a single passage through the liver, making it a key measure of hepatic drug clearance. It depends on hepatic blood flow, the drug’s unbound fraction, and the liver’s intrinsic metabolic and transport capacity, which determine how efficiently hepatocytes extract and process the compound. In clinical pharmacokinetics, this ratio helps classify drugs as having low, intermediate, or high hepatic extraction and predicts how liver disease, enzyme inhibition, altered blood flow, or changes in protein binding may affect drug exposure, dosing, and bioavailability.
