Diabetic Kidney Disease

Diabetic kidney disease is a progressive complication of diabetes in which chronic metabolic and vascular injury impairs the kidneys’ ability to filter blood, making it a major cause of chronic kidney disease and kidney failure. Persistent hyperglycemia alters glomerular cells and blood vessels, increasing intraglomerular pressure and disrupting the filtration barrier; albumin then enters the urine, while sustained injury reduces the glomerular filtration rate. In medicine, urine albumin and estimated filtration rate support early detection and monitoring, while glucose and blood-pressure management, renin–angiotensin system blockade, and newer kidney-protective therapies can slow progression. Understanding these mechanisms guides risk assessment and individualized care.

Our protocol was developed to cleanly and easily deliver islets or cells under the kidney capsule of mice. Cells are concentrated into pellets in the final tubing used for transplanting the cells under the kidney capsule. The ease of this technique reduces stress to the cells and the mouse.

The degree of vascular dysfunction and contributing physiological mechanisms can be assessed in patients with chronic kidney disease by measuring brachial artery flow-mediated dilation, aortic pulse-wave velocity, and vascular endothelial cell protein expression.
