Assisted Living

Assisted living is a residential care model that provides housing, daily support, and health-related services for people who need help with activities of daily living but do not require continuous skilled nursing. Care teams assess functional needs and develop individualized plans that may include medication assistance, meals, mobility support, personal care, safety monitoring, and coordination with clinicians. In clinical practice, assisted living supports older adults and people with chronic conditions while promoting autonomy, social engagement, and quality of life. Ongoing assessment helps identify changes in cognition, mobility, or health status and guides referrals, treatment adjustments, or transitions to higher levels of care.
