全南大学校病院実験動物研究センター倫理委員会(承認番号。CNU IACUC - H - 2022-36)は、すべての動物実験を承認しました。この研究で使用された雄のSprague-Dawleyラット(350-450 g)は、実験動物の世話と使用に関するガイドラインに従って世話を受けました。ラットは、標準的な餌と水が利用可能で、12時間の明暗サイクルで温度管理された部屋に収容されました。
1. 事前準備
注:1人の実験者がすべての実験手順を実行できます。
<ol><li>手術前に、酸素発生器、ポンプ、灌流ラインを含むランゲンドルフ装置を組み立てます(図2)。灌流回路に20 mLの生理食塩水を満たし、自家血でプライミングされるまで循環させます。 注意: このステップの目的は、体外回路を温めることです。</li>

</ol>2.ドナーの心臓温存と採血
<ol><li>麻酔室でイソフルラン(5%)を使用してドナーラットに麻酔を誘発し、手術台に置く前にラットの体重を記録します。.</li>
	<li>ラットを手術台の仰臥位に置き、ノーズコーンを通して2%〜2.5%のイソフルランと90%の酸素を送達することによって連続麻酔を投与する。</li>
	<li>つま先のつまみに対する反応の欠如と呼吸頻度(毎分50〜60の間)をチェックして、麻酔の深さを確認します。 注:ドナーラットへの不必要なストレスや痛みを避けるためには、適切なレベルの麻酔が重要です。</li>
	<li>目の潤滑剤を塗布し、手術が行われる鎖骨に恥骨領域を剃ります。ヨウ素ベースのスクラブと70%アルコールで領域をきれいにします。</li>

	</ol></li>

	</ol></li>
	<li>抽出<ol><li>すべてのCPSを投与した後、大動脈弓を周囲の組織から分離します。左鎖骨下動脈のすぐ下を注意深く解剖します。</li>
		<li>腕頭動脈と左総頸動脈を離れた位置で横断し、大動脈弓の長い切り株を残して、大動脈カニュレーション中の取り扱いを容易にします。主肺動脈(MPA)を分岐部にできるだけ近づけて横断します。左心房付属器を傷つけないように注意してください。</li>

</ol>3. その場 灌流
<ol><li>大動脈カニュレーションと灌流<ol><li>大動脈カニュレーションの前に、生理食塩水プライミング回路を血液プライミングに交換します。</li>

</ol>4.移植

	<li>腹部の臓器を温かく湿ったガーゼで包むことで脱水症を防ぎます。手術中、温かい生理食塩水を50 mLの注射器で断続的に広げます。</li>

Diclofenac, administered perioperatively (step 4.2.3.), enables pain management for the entire duration of this procedure. The analgesia regimen can be adjusted per the institutional animal use guidelines.</li>

Erratum: Rat Model of Normothermic Ex-Situ Perfused Heterotopic Heart Transplantation

私たちは、正常体温の上皮内灌流に関連する合併症を減らし、ドナー心臓の安全な保存時間を増やすためのモデルを開発しました。代替動物モデルと比較して、このモデルはシンプルで費用効果が高く、1人の実験者で実行できます。このモデルは、移植後のドナー心臓に対する薬理学的および遺伝的介入の長期的な有効性をテストするのに役立ちます。外科的処置には顕微手術のスキルが必要です。このモデルで成功を収めるための重要な要素は、原稿で概説されている重要なステップを実践し、集中することです。はじめに、酸素発生器、ポンプ、灌流ラインを含むランゲンドルフ装置を組み立てます。灌流回路に20ミリリットルの生理食塩水を満たし、自家血がプライミングされるまで循環させます。麻酔をかけたドナーラットで、7センチメートルの正中線の腹部切開を行い、次に剣状突起から鎖骨中央まで3センチメートルの両側切開を行います。胸部から毛皮を取り除きます。1ミリリットルの注射器を使用して、0.3ミリリットルの等張生理食塩水に溶解した1, 000国際単位のヘパリンを下大静脈から注入します綿棒で針穴をそっと圧縮して、針穴からの出血を止めます。5フレンチ大腿骨カテーテルを腹部大動脈に挿入します。カテーテルの先端が大動脈弓に到達していることを確認し、カテーテルの挿入部分のおおよその長さを評価してカテーテルの位置を確認します。腹部大動脈に挿入されたカテーテルを介して約10ミリリットルの血液を収集します。心臓麻痺液灌流ラインを腹部カテーテルに接続し、シリンジポンプで毎時800ミリリットルの速度で心筋麻痺液の投与を開始します。横隔膜から胸腔を開き、横隔膜に近い下大静脈を切断して心室の膨張を防ぎます。胸椎に沿って胸部の入口まで肋骨を両側に切断します。動員された腹側胸壁を蚊の鉗子で見越して反映します。すべての心麻痺溶液を投与した後、大動脈弓を周囲の組織から分離し、左鎖骨下動脈のすぐ下で慎重に解剖します。腕頭動脈と左総頸動脈を離れた位置で横断し、大動脈弓の長い切り株を残して、大動脈カニュレーション中の取り扱いを容易にします。次に、分岐部にできるだけ近い主肺動脈を横断します。上大静脈と下大静脈を5-0の絹縫合糸で慎重に結紮し、右心房と冠状動脈洞の閉塞を防ぎます。胸郭の左縁を湿ったガーゼで覆い、その上に心臓を置き、上大静脈と下大静脈結紮糸をそっと引っ込めて丘を露出させます。肺静脈とアジゴス静脈を5-0シルク縫合糸で縫合します。結紮糸の背側の組織を切断し、心臓を抽出します。怪我がないか調べた後、心臓の重さを量ります。生理食塩水プライミング回路を血液プライミングに交換し、毎分2〜3ミリリットルの流量で灌流を開始し、カニューレサイトから灌流液を漏らして気泡を除去します。永久結紮糸を配置した後、心臓が数秒以内に収縮し始め、60秒で正常なリズムに達することを確認します。55〜65ミリメートル水銀柱の平均灌流圧、摂氏37度で3〜4ミリリットルの冠状動脈流量は、適切な灌流を示します。リザーバーから0.15ミリリットルの血液を採取し、灌流開始時とその後20分ごとに血液ガス分析を確認します。120分の灌流後、3ミリリットルのクストジオールをシリンジポンプを通して毎時250ミリリットルの速度で投与し、心臓を逮捕する。正中線開腹術を行い、開創器を挿入して腹腔を広げます。綿棒を使用して腹部臓器をレシピエントの左側に動員し、手順のためのスペースを確保します。倍率10倍の手術用顕微鏡を利用して、綿棒による鈍的解剖により十二指腸と近位空腸を動員します。腹部大動脈と下大静脈を露出させる。主要な血管の頭蓋側と尾側の両方の腸間膜枝に5-0の絹結紮糸を置きます。腹部血管を持ち上げ、5〜0本の絹縫合糸で腰椎枝を凝固または結紮します。精巣動脈と静脈を惜しまないことを忘れないでください、そしてそれらを固定しないでください。結紮糸を使用して血管を持ち上げ、マイクロクランプを主要血管の腸間膜枝、尾側、頭蓋側に配置して、吻合部位の血流を停止します。27ゲージの針を使用して大動脈を穿刺し、マイクロハサミで切開部をドナー上行大動脈の開口部と同じかそれよりわずかに大きい長さまで伸ばします。大動脈切開と同じ要領で下大静脈を縦方向に切開しますが、大動脈切開に比べて尾側に3mm近づけます。吻合を開始し、ドナーの心臓をレシピエントの腹部の右側に置き、ドナーの上行大動脈をレシピエントの腹部大動脈に取り付け、縦切開の頭蓋角に1つの簡単な中断ステッチを付けます。心臓をレシピエントの腹部の左側に移動し、ランニング9-0ポリプロピレン縫合糸を使用して、ドナーの上行大動脈とレシピエントの腹部大動脈の吻合を行います。ドナー肺動脈を下大静脈に固定し、縦切開の尾側と頭蓋の角に2つの中断された縫合糸を付けます。吻合が完了したら、尾クランプと頭蓋クランプを取り外します。適切な止血を保証し、蛇行したように腹部臓器を交換し、連続した5-0ポリプロピレン縫合糸を使用して腹部切開の層を閉じます。乳酸塩、カリウム、平均大動脈圧など、ex situ灌流中の心臓の生存率を評価するために使用されるパラメーターをこの図に示します。ここで、正常体温上皮内温存の使用により、成功した6例の総虚血時間は46.2プラスマイナス4.7分に減少し、体外出時間の合計は166.2プラスマイナス4.7分でした。ドナーからの心臓の摘出およびex-situ灌流および異所性移植の準備に必要なものは、5.8プラスマイナス1.3分であった。手術の成功率は70%であり、成功した6例の平均吻合時間は38.4プラスマイナス3.4分でした。調達前および移植後のドナー心臓の電気生理学的性能をこの図に示します。すべての実験において、心拍数は移植直後に有意に減少したが、最終的には時間の経過とともに回復した。ここでは、正常体温外その場保存前、正常発熱上皮内保存後、および異所性移植の2時間後のドナー心臓の巨視的および顕微鏡的外観を示します。この手順を行うときは、灌流に十分な血液を採取し、上行部分を損傷することなく大動脈を慎重に切断し、保存中の生理学的パラメータを注意深く観察することが重要です。この手順に続いて、新しい保存液の有効性と、新しく導入された保存方法に対するレシピエントの免疫応答をテストできます。ex-situ灌流と異所性移植を組み合わせることで、保存期間を長くすることで新しい技術を検討し、レシピエントの免疫応答に対する長期的な影響を観察することができます。
