馬の腹部の外科的閉鎖、切開性合併症の予防と管理

Surgical Closure of Equine Abdomen, Prevention, and Management of Incisional Complications

腹部切開の閉鎖と術後管理のための日常的な外科的技術: 私たちの linea alba の閉鎖と日常的な術後管理の技術は、2020 年の発表以来、わずかな適応しか受けていません3.
したがって、腹痛のために手術を受けた馬の場合、ここで提示する結果は、研究期間中に疝痛の徴候を示す 564 頭の馬で実施された 606 回の開腹術を含む、このレトロスペクティブ研究³の結果です。開腹切開の平均長さは 17.5 cm でした。この発表では、単回開腹術後に9.5%(52/546)の切開合併症が明らかになり、再開腹術後に33.3%(10/30)が明らかになりました。切開合併症は、血清性分泌物による漿液性(n=19)、血腫(n=3)、副鼻腔縫合管/限局性感染症(n=9)、切開部の1/3以上にわたる表在性感染(n=27)、深部切開感染と敗血症性腹膜炎による完全創傷破壊(n=1)、転倒による急性 白濁 線および皮下裂開(n=1)、感染を伴わないヘルニア形成(n=2)であった。したがって、以前の研究 3 では、単回開腹術と反復開腹術後の実際の切開感染の発生がそれぞれ 5.3% と 26.7% であると報告されています。

この研究3 の長期追跡調査(術後12週間≥;n=417)では、臨床的に関連性のある創傷合併症の1.68%が明らかになった:4例で小さなヘルニア(直径≤5cm)、3例で大きなヘルニア。
帝王切開の結果については、2013年1月1日から2022年12月31日までの間に帝王切開を受けた牝馬の医療記録をレビューし、疝痛馬の研究と同じ選択基準を使用しました3(すなわち、術後7日間の生存≥腹側腹壁の既存の病変がない)。私たちの帝王切開の症例数は、疝痛手術の症例数と比較して非常に低いです。選択基準を満たしたのはわずか12頭(体重中央値:636kg)でした。開腹切開の平均長さは38cmでした。切開合併症は、4日以内に解消した漿液性分泌物(n = 1)、副鼻腔縫合管/限局性感染症(n = 1)、部分的な白杉、および麻酔からの激しく非常に長い回復による皮下裂開(n = 1)でした。この馬は再手術(すべての切開層を完全に閉鎖)され、表在性創傷感染症を発症しました。術後3ヶ月の再評価では、感染は解消し、腹部ヘルニアは認められませんでした。
閉鎖切開部での陰圧療法の使用(感染予防のため): この技術によって治療された症例の中で、この治療法の潜在的な有効性を評価するために、選択基準と除外基準を決定しました (2020 年 1月 1 日から 3 年間)。
包含基準: 感染のリスクが高いと考えられる症例 (早期開腹術 - つまり、最初の手術後 7 日以内) または開腹切開が 30 cm を超える)、術後少なくとも 7 日間の生存。麻酔からの回復直後の陰圧療法の適用。
除外基準:小動物(ロバとポニー&lt;250 kg)、1歳未満の子馬。
合計8頭の馬が選択基準を満たしました(早期開腹術6頭と長切開術2例)。そのうち、5人は切開合併症を経験しておらず(開腹術4回、長切開1回)、他の3人は切開感染症を発症しました。同じ期間に、4頭の馬が最初に選択基準を満たしていましたが、システムの正しい適用と機能を妨げる激しい痛みや気難しい気質のために除外されました。そのうち、3つの開腹術のうち1つと1つの長い切開が感染しました。
結果は2つのグループ間で異なるようには見えませんが、グループのサイズが統計分析を妨げ、システムの非有効性をより大きなグループで評価および確認する必要があります。システムの適用が遅れたり、プロトコルに記載されているもの以外の適応症(重度の浮腫、汚染された手術など)など、さまざまな理由で含まれなかった動物の結果は、臨床医に全体的に肯定的な印象を残しました。
感染した開腹切開の治療のための陰圧療法の使用: 過去 11 年間で、50 頭の馬の感染した開腹切開または再発性漿液腫の治療に陰圧療法が適用されました。管理の関連プロトコルは、過去 3 年間ヘルニア ベルトを使用し、昨年から運動のオプション プロトコル (Holcombe et al.19 と Clayton20 から適応) の導入によって洗練されました。
ほとんどの場合、局所療法は7〜10日間適用されました。対照群はなかったが(陰圧療法を併用しなかった場合)、肉芽組織の形成を促進し、分泌物の連続吸引により包帯の浸漬を減少させるという肯定的な臨床的印象を受けた。数頭の馬は、陰圧療法が中止された後、数週間の間、残留した化膿性分泌物のみを産生し続けました。.
NPWTによって治療された感染性開腹切開の進化は、2頭の馬について示されています(馬1:図2A-C;馬2:図3A-B)。

この治療法は、重度の感染症を経験した3頭の馬の切開の完全な崩壊を防ぐのに十分ではありませんでした。そのうち2人は安楽死させられ、1人はその後、上記の保持縫合糸で手術を受けました。さらに、一部の馬は、陰圧療法を受けている間に他の合併症(すなわち、再発性の激しい痛みなど)のために安楽死させられ、感染した開腹切開術でのこの治療の評価が不完全になりました。
数頭の馬は、切開感染後に腹側腹部の軽度から中等度の変形を発症しました。しかし、保持縫合糸で治療された馬を除外すると、陰圧療法とヘルニアベルトで治療された馬はいずれも矯正手術を必要とする重大な腹部ヘルニアを発症しませんでした。
保持縫合糸による完全な創傷破壊の管理: 文献6,9に記載されているように、開腹切開の完全な崩壊はまれです。ここで説明した症例(馬、去勢馬、ザンガーハイデ、492 kg)は最初の開腹術を受け、その間に腸チフス切開と乾燥盲腸内容物の手動排出が行われ、重度の盲腸の宿便(小腸捻転に加えて)が矯正されました。感染症の発症後、開腹切開術に陰圧療法が適用されましたが、馬は切開の進行性の崩壊を示し、上記の技術に従って再手術されました。手術中、盲腸体と開腹切開部との間の癒着、および盲腸尖と右腹側結腸との間の癒着が検出され、切除されました。クリニックでの術後期間は平穏でした。
馬は術後15ヶ月で再検査されました。騎手によると、馬がヘルニアベルトの腹側ポケットに配置されなかった腹側補強(湾曲したプラスチック片)を除いて、クリニックからの退院後にすべての推奨事項が守られました。手術の後遺症は、以前の保持縫合糸の位置にある瘢痕といくつかの小さなヘルニアであり、馬の美容的外観を大幅に低下させます。しかし、馬は元のスポーツパフォーマンスのレベル(120cm跳躍)に完全に回復し、さらに改善するためにまだ訓練されています。退院以来、術後疝痛の徴候は報告されていません。
