Patient Assessment

Patient assessment is the systematic collection and interpretation of information about a person's health, providing the foundation for clinical decision-making and individualized care. Clinicians combine a health history, physical examination, vital signs, and available diagnostic findings to identify symptoms, recognize abnormal patterns, and evaluate changes over time. In clinical practice, assessment helps establish priorities, support diagnosis, identify risks, and guide treatment planning. Repeated assessments also allow healthcare professionals to monitor responses to interventions, communicate patient status, and adjust care as conditions evolve.
