Biopsy Needle Guidance

Biopsy needle guidance is the use of imaging and procedural techniques to direct a needle accurately to a suspicious tissue abnormality for sample collection. Ultrasound, computed tomography, magnetic resonance imaging, or other guidance methods provide anatomical visualization, allowing clinicians to plan the needle trajectory, monitor its advancement, and position the tip within the target while avoiding nearby structures. In medicine, guided biopsy supports minimally invasive diagnosis of tumors, inflammatory lesions, and other abnormalities, often reducing the need for open surgery. Accurate targeting can improve specimen quality, inform treatment decisions, and enhance procedural safety.

Substernal thyroid lesions are common, and need to be differentiated from malignancy. Obtaining percutaneous fine needle biopsy is not possible due to its retrosternal location. This article proposes a protocol for biopsy of substernal thyroid lesions using Endobronchial Ultrasound-guided Transbronchial Needle Aspiration (EBUS-TBNA).
