<ol><li>(1) 병리학적으로 확인된 갑상선암이 있고 수술적 치료가 필요한 경우 환자를 포함합니다. (2) T3-T4(American Joint Committee on Cancer TNM 병기 결정, 8판)와 같은 광범위한 국소 종양 침윤이 있거나 전이성 병변이 기관, 식도 및 큰 혈관과 같은 중요한 구조를 침범한 경우; (3) 본인과 가족 구성원이 컴퓨터 지원 3D 시각화에 자원합니다. (4) 마취에 대한 금기 사항이 없었다.</li>

</ol>2. 이미징 획득
<ol><li>256층 나선형 CT 시스템을 사용하여 환자의 일반 및 강화 CT(정맥 및 동맥기 포함) 이미지를 얻습니다. 스캐닝 매개변수는 120kV, 120mA, 512 x 512 매트릭스, 0.625mm 층 두께, 150HU 임계값 및 10-20초 동맥 스캔 지연입니다.</li>

</ol>3. 컴퓨터 지원 3D 모델링
<ol><li>데이터를 3D 시각화 소프트웨어로 가져옵니다(그림 1A).<ol><li>열기 버튼을 클릭하여 DICOM 형식의 환자 데이터가 포함된 문서를 선택합니다. 데이터를 소프트웨어로 가져옵니다.</li>

갑상선암은 중국에서 7번째로 흔한 악성종양이며1 수술이 가장 중요한 치료 방법이다 2,3. 종양의 완전한 절제는 국소 진행성 갑상선암 환자의 높은 생존율과 좋은 삶의 질과 밀접한 관련이 있다 3,4; 그러나 이러한 유형의 절제술은 쉽지 않습니다. 목에는 기관, 식도, 총경동맥과 같은 중요한 장기와 조직이 있습니다. 진행성 갑상선암의 절제술은 이러한 종양이 목과 종격동의 중요한 장기와 큰 혈관에 근접해 있다는 점을 고려할 때 훨씬 더 위험하고 어렵다 5,6. 따라서 적절한 수술 전 평가가 필요합니다.
현재 임상 환경에서 널리 사용되는 컴퓨터 단층 촬영(CT), 자기 공명(MRI) 및 컬러 도플러 초음파는 2차원(2D) 보기를 제공하여 종양 부피, 경계 및 중요한 주변 구조와의 관계에 대한 평가를 제한합니다 7,8. 외과의가 2D 이미지를 3D 공간으로 변환하기 전에 상당한 임상 경험과 효율적인 시행착오가 필요합니다. 컴퓨터 지원 3D 시각화는 2D 이미징을 사용하여 수술 전 계획 및 치료 계획 선택에 사용할 수 있는 보다 직관적인 3D 모델을 생성함으로써 의사-환자 커뮤니케이션을 보다 직관적으로 만들고 의사-환자 의견 불일치를 줄일 수 있습니다. 이 모델은 3D 시각화를 제공하지만 무형입니다. 이 3D 가이드 수술 전 평가 및 준비는 수술 시간을 단축하고 수술 위험을 줄일 수 있습니다. 3D 접근법은 간담도 수술, 정형외과, 구강악안면 수술에서 널리 사용되어 왔다 9,10. 갑상선암에서 3D 시각화는 현재 초음파 진단 및 수술 계획 수립을 지원하는 데 사용됩니다 11,12,13,14,15.

Computer-Aided Three-Dimensional Visualization in the Treatment of Locally Advanced Thyroid Cancer

2017년 12월부터 2021년 7월까지 국소 진행성 갑상선암 환자 23명을 대상으로 3D 모델링을 진행했습니다. 이 23명의 환자 중 4명은 수술 위험성으로 인해 수술 대상에서 제외되었고, 나머지 19명은 3D 모델링에 따른 수술로 치료되었다(표 1). 19명의 환자는 모두 국소 갑상선암을 앓고 있었으며, 이 중 14명은 최초 진단을 받았고, 16명은 호흡곤란의 정도가 다양했으며, 18명은 목에 큰 종양(원발성 갑상선 종양 또는 전이성 림프절)이 있어 주변 조직을 침범했다. 수술 후 병리학적 평가에서 11명의 환자가 분화성 갑상선암, 2명은 갑상선 수질암, 5명은 미분화 또는 잘 분화되지 않은 갑상선암, 1명은 랑게르한스 세포 조직구증을 동반한 갑상선 유두암으로 밝혀졌다. 수술 전 3D 모델링을 사용하여 의사와 환자의 효율적인 커뮤니케이션을 촉진했습니다. 모든 수술이 성공적으로 끝났고, 수술 후 회복은 순조로웠으며, 수술 전후 사망자는 없었습니다.
