본연구3 의 장기 추적 관찰(≥ 수술 후 12주, n=417)에서 임상적으로 관련된 상처 합병증이 1.68%를 나타냈다: 4례에서 작은 탈장(직경 5cm ≤), 3례에서 큰 탈장.
제왕절개 결과의 경우, 2013년 1월 1일부터 2022년 12월 31일까지 제왕절개 수술을 받은 암말의 의료 기록을 검토하고, 배앓이 말3에 대한 연구와 동일한 선정 기준(즉, 수술 후 7일 ≥ 생존하고 복부 복벽에 기존 병변이 없는 경우)을 사용했다. 우리의 제왕절개 사례는 배앓이 수술 사례에 비해 매우 적습니다. 12마리의 암말(중간 체중: 636kg)만이 포함 기준을 충족했습니다. 개복술 절개 부위의 평균 길이는 38cm였다. 절개 합병증은 다음과 같습니다: 4일 이내에 해결된 장액성 분비물(n=1), 부비동 봉합사/국소 감염(n=1), 부분적인 알바 선, 격렬하고 매우 긴 마취 회복으로 인한 피하 박리(n=1). 이 말은 재수술(모든 절개층의 완전한 봉합)을 받았고 표재성 상처 감염이 발생했습니다. 수술 3개월 후 재평가에서 감염은 사라졌고 복부 탈장은 없었다.
폐쇄 절개 부위에 음압 요법 사용(감염 예방을 위해): 임상에서 이 기법으로 치료한 사례에서 이 요법의 잠재적 효능을 평가하기 위해 포함 및 제외 기준을 결정했습니다(2020년 1월 1일부터 3년 동안).
포함 기준: 감염 위험이 더 높은 것으로 간주되는 사례(조기 복강 절개술 - 즉, 첫 번째 수술 후 7일 이내 또는 30cm 이상의 절개 개복술) 및 수술 후 최소 7일 동안 생존합니다. 마취에서 회복 직후 음압 요법을 적용합니다.
제외 기준: 작은 동물(당나귀 및 조랑말 &lt; 250kg), 1세 미만의 망아지.
총 8마리의 말이 포함 기준을 충족했습니다(6마리의 조기 복교 절개술과 2마리의 긴 절개). 그 중 5명은 절개 합병증이 없었고(4명은 복강절개술, 1명은 장절개술), 나머지 3명은 절개 감염이 발생했다. 같은 기간 동안 4마리의 말이 처음에는 포함 기준을 충족했지만 심한 통증이나 불안정한 기질로 인해 시스템의 올바른 적용과 기능을 방해하여 제외되었습니다. 그 중 복강절개술 3건 중 1건과 긴 절개 부위 1건 중 1건이 감염되었습니다.
두 그룹 간에 결과가 다르지 않은 것으로 나타나지만, 그룹의 규모로 인해 통계 분석이 불가능하고 시스템의 비효율성은 더 큰 그룹으로 평가하고 확인해야 합니다. 시스템의 늦은 적용 또는 프로토콜에 나열된 것 이외의 적응증(예: 심각한 부종, 오염된 수술 등)과 같은 다양한 이유로 포함되지 않은 동물에 대한 결과는 임상의에게 전반적으로 긍정적인 인상을 남겼습니다.
감염된 개복술 절개 부위 치료를 위한 음압 요법 사용: 지난 11년 동안 50마리의 말에서 감염된 개복술 절개 부위 또는 재발성 혈청종 치료를 위해 음압 요법이 적용되었습니다. 관련 관리 프로토콜은 지난 3년 동안 탈장 벨트를 사용하고 작년부터 선택적 운동 프로토콜(Holcombe et al.19 및 Clayton20에서 수정)을 도입함으로써 개선되었습니다.
대부분의 경우 음압 요법은 7-10일 동안 적용되었습니다. (음압 요법을 사용하지 않고) 대조군은 없었지만, 지속적인 분비물 흡인으로 육아 조직 형성을 가속화하고 붕대가 적시는 것을 줄인다는 긍정적인 임상적 인상을 받았습니다. 몇몇 말은 음압 요법이 중단된 후 몇 주 동안 잔류 화농성 분비물만 계속 생성했습니다.

이 치료법은 심각한 감염을 경험한 3마리의 말에서 절개 부위가 완전히 파괴되는 것을 예방하기에 충분하지 않았습니다. 그 중 2명은 안락사되었고, 1명은 그 후 위에서 설명한 고정 봉합사로 수술을 받았습니다. 더욱이, 일부 말은 음압 요법을 받는 동안 다른 합병증(예: 재발성 심한 통증 등)으로 인해 안락사되어 감염된 개복 절개 부위에 대한 이 요법에 대한 평가가 불완전했습니다.
몇몇 말들은 절개 감염 후 복부 복부에 경증에서 중등도의 기형을 보였다. 그러나 유지 봉합사로 치료한 말을 제외했을 때, 음압 요법과 탈장 벨트로 치료받은 말 중 교정 수술이 필요한 심각한 복부 탈장이 발생한 말은 없었다.
유지 봉합사에 의한 완전한 상처 파괴 관리: 문헌 6,9에 기술되어 있듯이, 개복술 절개 부위가 완전히 파괴되는 경우는 드뭅니다. 여기에 설명된 사례(horse, gelding, Zangersheide, 492kg)는 첫 번째 개복술을 받았고, 이 과정에서 심각한 두칼 매복(소장 volvulus 외에)을 교정하기 위해 발진형 절개술과 건조한 세칼 함량의 수동 배출이 수행되었습니다. 감염이 발생한 후 개복술 절개 부위에 음압 요법이 적용되었지만, 말은 절개 부위가 점진적으로 파괴되었고 위에서 설명한 기술에 따라 재수술을 받았습니다. 수술 중 소골체와 개복술 절개 사이, 소골 정점과 오른쪽 복부 결장 사이의 유착이 감지되고 절제되었습니다. 수술 후 클리닉에서의 기간은 별다른 일이 없었습니다.
말은 수술 후 15개월 후에 재검사를 받았습니다. 기수에 따르면, 클리닉에서 퇴원한 후 말이 약간의 불편함을 보였기 때문에 탈장 벨트의 복부 주머니에 넣지 않은 복부 보강재(구부러진 플라스틱 조각)를 제외한 모든 권장 사항을 따랐습니다. 수술의 후유증은 흉터와 이전 고정 봉합사 위치에 있는 여러 개의 작은 탈장으로 말의 미용적 외관을 크게 떨어뜨립니다. 그러나 말은 원래 스포츠 성능 수준(120cm 점프)으로 완전히 회복되었으며 여전히 더 향상되기 위해 훈련을 받고 있습니다. 병원에서 퇴원한 이후 수술 후 배앓이 징후는 보고되지 않았습니다.
