Antibiotic Susceptibility Testing: Epsilometer Tests to Determine MIC Values of Two Antibiotics and Evaluate Antibiotic Synergy

1. Epsilometer testen (E-tests)

		<li>Bewaar de McFarland turbiditeitsstandaard Nr. 0,5 in een met folie bedekt buisje. Bewaar bij 25°C gedurende maximaal 6 maanden. Vorm goed tot een homogene oplossing voor gebruik.</li>

	<li>Bereiding van MHA platen
	<ol>
		<li>Schraap Streptococcus groep G bacteriën van een bloedagar plaat met een steriele lus. Meng in 1mL zoutoplossing en vorm tot een suspensie van de bacteriën.</li>
		<li>Vergelijk de suspensie met een McFarland standaard Nr. 0,5 om dezelfde turbiditeit te bereiken om dezelfde inoculum grootte te hebben tijdens de experimenten. Pas de concentratie aan met behulp van extra zoutoplossing of bacteriën.</li>
		<li>Inoculeer de MHA platen door een steriele katoenen applicator te gebruiken. Veeg de plaat voorzichtig om het oppervlak te bedekken. Ga verder met een van de drie onderstaande methoden (1.4-1.6).</li>

Kennis van de interacties tussen antibiotica en bacteriën is belangrijk om te begrijpen hoe microben antibioticaresistentie ontwikkelen. In 1928 ontdekte Alexander Fleming penicilline, een antibioticum dat zijn antibacteriële functie uitoefent door interferentie met celwandregeneratie (1). Andere antibiotica met verschillende werkingsmechanismen zijn vervolgens ontdekt, inclusief geneesmiddelen die DNA-replicatie en eiwittranslatie in bacteriën remmen; er zijn echter geen nieuwe antibiotica ontwikkeld in recente jaren. De resistentie tegen huidige antibiotica is toegenomen, wat leidt tot ernstige infectieziekten die niet effectief behandeld kunnen worden (2). Hier beschrijven we verschillende methoden om antibioticaresistentie in bacteriepopulaties te beoordelen. Elk van deze methoden werkt, ongeacht het werkingsmechanisme van de gebruikte antibiotica, omdat bacteriële dood het gemeten resultaat is. Antibioticaresistentie wordt niet alleen snel verspreid in specifiek ziekenhuisomgevingen, maar ook in de hele samenleving. Om dergelijke manieren van resistentie te onderzoeken, zijn verschillende methoden ontwikkeld, waaronder de Epsilometer-test (E-test) en de bouillonverdunningstest (3).
De E-test is een goed gevestigde methode en is een kosteneffectief hulpmiddel dat Minimale Remmende Concentratie (MIC) gegevens kwantificeert, de laagste concentratie van een antimicrobieel dat zichtbare groei van een micro-organisme remt. Afhankelijk van de bacteriestam en gebruikte antibiotica kan de MIC-waarde variëren tussen sub μg/mL tot >1000 μg/mL (4). De E-test wordt uitgevoerd met behulp van een plastic strip met een vooraf gedefinieerde antibioticagradiënt, die is bedrukt met de MIC-leesschaal in μg/mL. Deze strip wordt direct overgebracht op de agarmatrix wanneer toegepast op de geïncubeerde agarplaat. Na incubatie is een symmetrische elliptische remmingszone zichtbaar langs de strip, omdat bacteriegroei wordt voorkomen. MIC wordt gedefinieerd door het remmingsgebied, dat is het eindpunt waar de ellips de strip kruist. Een andere veelgebruikte methode om MIC te bepalen is de microbroth verdunningsmethode. Microbroth verdunning bevat verschillende concentraties van het antimicrobiële middel toegevoegd aan een bouillonmedium met geïncubeerde bacteriën. Na incubatie wordt MIC gedefinieerd als de laagste concentratie van antibioticum die zichtbare groei voorkomt (5). Het is ook een kwantitatieve methode en kan op verschillende bacteriën worden toegepast. Nadelen van deze methode zijn de mogelijkheid van fouten bij het bereiden van de concentraties van de reagentia en het grote aantal reagentia dat voor het experiment vereist is. Het meten van antibioticaresistentie is zowel vanuit klinisch als onderzoeksperspectief cruciaal, en deze in vitro methoden om resistentie te onderzoeken worden hieronder besproken en gepresenteerd.
Het resistieprofiel voor een specifiek bacterie kan worden toegepast om antibioticabehandeling te optimaliseren om te bepalen of een patiënt zou profiteren van combinatiebehandeling versus enkele therapie. Voor het gebruik van meer dan één antibioticum op een enkele tijd is het van essentieel belang om hun interacties met elkaar te kennen en of ze een additief, synergistisch of antagonistisch effect hebben. Een additief effect kan worden gezien wanneer het gezamenlijke effect van de antibiotica gelijk is aan de potentie van de individuele antibiotica gegeven in een gelijke dosis. Synergie tussen antibiotica is daarentegen aanwezig wanneer het gezamenlijke effect van de antibiotica krachtiger is dan wanneer het medicijn alleen zou worden gegeven (6). Het toepassen van combinaties van antimicrobiële behandeling wordt gebruikt om de voorkoming van antimicrobiële resistentie te voorkomen en zo het effect van de individuele antibioticabehandeling te verbeteren (7). Kennis van antagonisme is ook net zo belangrijk om onnodig gebruik van antimicrobiële combinaties te voorkomen. E-test methodologie biedt eenvoudige en verschillende manieren om mogelijke synergie en antagonisme tussen verschillende antimicrobiële middelen te bepalen. Om de proliferatie van antibioticaresistente pathogenen tegen te gaan, is kennis van mogelijke synergistische en antagonistische mechanismen van bepaalde antibiotica belangrijk, resulterend in klinische effectiviteit en het bestrijden van multidrug-resistentie.
De bepaling van synergie met behulp van E-tests kan worden verdeeld in twee brede benaderingen: kruis- en niet-kruistesten. Hoewel beide synergietesten berusten op de eerdere kennis van individuele MIC-waarden, verschillen de twee benaderingen enigszins in methodologie en conceptuele benadering. Bij een niet-kruissynergietest wordt het eerste antibioticum in het te testen paar op een met bacteriën geïncubeerde agarplaat geplaatst. Na het laten infuseren van de antibiotica van de eerste strip in de plaat (bijv. na 1 uur), wordt de strip verwijderd en wordt een nieuwe strip met het tweede antibioticum op dezelfde plaats als de eerste geplaatst, ervoor zorgen dat de twee individuele MIC-waarden boven elkaar worden geplaatst. De resulterende remmingszone kan vervolgens worden geanalyseerd zoals hierboven beschreven, en synergie kan worden berekend op basis van vergelijking 1.

Hoewel de onderzoeker wordt beloond met gemakkelijk te analyseren platen, is de methode enigszins arbeidsintensief en tijdrovend vanwege het wisselen van strips, evenals de noodzaak om twee platen per experiment te gebruiken. Gevoeligheid voor antibiotica wordt gedefinieerd als de gevoeligheid van een bacterie voor antibiotica en kan worden gemeten met behulp van een bouillonverdunningstest of een Epsilometer-test, ook wel E-test genoemd.In de bouillonverdunningsmethode wordt een gestandaardiseerd aantal bacteriën toegevoegd aan een groeimedium dat seriële antibioticadiluties bevat. Als ze gevoelig zijn, kunnen de bacteriën niet groeien bij de hogere antibioticumconcentraties, maar blijven ze zich vermenigvuldigen bij de lagere antibioticumconcentraties, waardoor het medium troebel wordt. De laagste antibioticumconcentratie waarbij de bacteriën niet langer kunnen overleven of zich vermenigvuldigen, wordt de minimale remmende concentratie of MIC-waarde van het antibioticum voor de gegeven bacterie genoemd.Bij een E-test wordt een plastic strip geïmpregneerd met een vooraf gedefinieerde antibioticumgradient aangebracht over een vers verspreide laag bacteriën op een Mueller-Hinton-agar, of MH-A, Petri-schaal. Het antibioticum verspreidt zich in het agarmedium, waar het wordt opgenomen door de bacteriën. Als ze gevoelig zijn, kunnen de bacteriën zich niet vermenigvuldigen en zullen ze afsterven, waardoor een heldere zone rond de E-strip ontstaat, die de zone van groeiremming wordt genoemd. Op het punt waar de groei de E-strip kruist, geeft de overeenkomstige waarde op de schaal de MIC-waarde van het antibioticum.Vaak worden antibiotica in combinatie gebruikt om het ontstaan van antibioticaresistente bacteriestammen te voorkomen. Dit resulteert vaak in een synergistisch, in plaats van additief, effect. Synergetisch betekent dat het gecombineerde effect van de twee antibiotica groter is dan de som van hun individuele activiteiten. De effect wordt echter alleen significant geacht wanneer de MIC-waarde van de antibioticumcombinatie met ten minste de helft daalt. Dit criterium wordt geëvalueerd door de fractionele remmende concentratie, of FIC, index te berekenen. Door de verhouding van de MIC van elk antibioticum in combinatie met de MIC van elk antibioticum afzonderlijk op te tellen, geeft een FIC-index van minder dan 0,5 synergie aan.Antibioticumsynergie kan worden gemeten met behulp van twee op E-test gebaseerde methoden: een niet-kruistest of een kruistest. Bij een niet-kruistest worden eerst de E-strips voor twee verschillende antibiotica met vooraf bepaalde MIC-waarden aangebracht op twee afzonderlijke schalen. Nadat de antibiotica in het medium zijn gediffundeerd, worden de oorspronkelijke E-strips verwijderd en worden de E-strips voor de alternatieve antibiotica zo geplaatst dat hun MIC-schalen precies over de MIC-schalen van de vorige strips liggen. Bij een kruistest, een snellere versie van de niet-kruistest, worden de E-strips van de twee antibiotica samen in een kruisvorm geplaatst, zodat de schalen van hun MIC-markeringen een hoek van 90 graden vormen op het kruispunt. Na incubatie in beide technieken wordt de MIC-waarde van elk antibioticum in combinatie met het andere antibioticum gelezen op het punt waar de zone van groeiremming de rand van de E-strip kruist. Plaats nu de bacteriesuspensie en McFarland-troebelheidsstandaarden naast elkaar en vergelijk ze op troebelheidsequivalentie. Voeg ofwel extra zoutoplossing of bacteriekolonies toe tot de troebelheid van de bacteriesuspensie overeenkomt met die van de standaard. Zodra de gewenste troebelheid is verkregen, doopt u een steriele katoenen puntapplicateur in de bacteriesuspensie. Om de MH-A-plaat te inoculeren, veegt u de volledige oppervlakte van de plaat voorzichtig met een zigzagbeweging af. Label vervolgens de onderkant van de platen met de naam van de bacterie en de datum.Om te beginnen, haalt u een penicilline G E-teststrip uit, houdt u deze met behulp van een pincet bij de rand vast. Plaats de strip voorzichtig in het midden van de vers geveegde MH-A-plaat en plaats het deksel er weer op. In dit voorbeeld wordt ook een tweede antibioticum, gentamicine, getest. Daarom wordt het stripplaatsingsproces herhaald met de tweede plaat en een gentamicine E-teststrip. Om de resultaten van de E-test te bepalen, verzamelt u de eerste plaat die de penicilline G E-teststrip bevat. Bepaal nu het punt waar de remmingszone de antibioticumstrip kruist. Lees de bijbehorende numerieke waarde op de schaal. Deze waarde vertegenwoordigt de MIC-waarde van penicilline G. Bepaal de MIC-waarde voor gentamicine op dezelfde manier.Om te beginnen, inoculeert u een MH-A-plaat met Streptococcus groep G-stambacteriën. Measuring bacterial resistance with E-tests helps show how strongly an antibiotic can stop growth. The method is used to find the minimal inhibitory concentration, or MIC, which is the lowest antibiotic level that prevents visible bacterial growth. It is a practical way to study resistance in bacteria and compare how different antibiotics perform.

The E-test uses a plastic strip that contains a built-in antibiotic gradient. The strip is placed on an agar plate that has been inoculated with bacteria, and the MIC scale is printed directly on the strip in micrograms per milliliter. After incubation, an ellipse-shaped zone appears where growth is blocked. The MIC is read where that inhibition zone meets the strip.

A similar MIC method is microbroth dilution. In this approach, bacteria are placed in broth with different antibiotic concentrations. After incubation, the MIC is the lowest concentration that still stops visible growth. This method is quantitative and can be used with several bacteria, although it can require many reagents and careful preparation of the concentrations.

Knowing a bacterium’s resistance profile can help guide treatment choices. It can show whether a patient may benefit from one antibiotic or from a combination. When two antibiotics are used together, they may act in an additive, synergistic, or antagonistic way. Additive means the combined effect matches the effect of each drug at equal dose, while synergy means the combined effect is stronger than either drug alone.

E-tests can also be used to look for synergy and antagonism between antimicrobial agents. One method is non-cross testing. In this setup, one antibiotic strip is first placed on the agar plate, allowed to diffuse, and then removed. A second strip is placed in the same position, with the two MIC values aligned, and the result is used to calculate the fractional inhibitory concentration, or FIC. Values above 0.5 show synergy. This method is simple to read, but it takes more time and uses two plates.

A faster option is the cross test. Here, both E-test strips are placed at the same time in a 90-degree cross, with the known MIC values meeting at the angle. Only one plate is needed, so the method saves work, although the result can be a little harder to analyze. The changed inhibition zones are then used to determine whether the antibiotics act synergistically.

A broth-based approach can also be used for combination testing. It gives more flexibility because specific antibiotic concentrations can be chosen outside the limits of an E-test strip. It may also be more sensitive because the antibiotics are evenly distributed in liquid, rather than relying on diffusion through agar. In this method, wells in a 96-well plate are filled with a set number of bacteria and different antibiotic dilutions, and the MIC is the lowest concentration that prevents visible growth.
