Rattenmodel van Normothermische ex-situ perfuseerde heterotope harttransplantatie

Samenvatting

Hier presenteren we een beoordelingsprotocol van een heterotopisch geïmplanteerd hart na normotherm ex situ behoud in het rattenmodel.

Samenvatting

Harttransplantatie is de meest effectieve therapie voor eindstadium hartfalen. Ondanks de verbeteringen in therapeutische benaderingen en interventies, neemt het aantal patiënten met hartfalen dat wacht op transplantatie nog steeds toe. De normotherme ex situ conserveringstechniek is vastgesteld als een vergelijkbare methode als de conventionele statische koelopslagtechniek. Het grote voordeel van deze techniek is dat donorharten tot 12 uur in fysiologische toestand kunnen worden bewaard. Bovendien maakt deze techniek reanimatie van de donorharten na circulatoire sterfte mogelijk en past de vereiste farmacologische interventies toe om de donorfunctie na implantatie te verbeteren. Er zijn talloze diermodellen opgesteld om normotherme ex situ conserveringstechnieken te verbeteren en conserveringsgerelateerde complicaties te elimineren. Hoewel grote diermodellen gemakkelijk te hanteren zijn in vergelijking met kleine diermodellen, is het duur en uitdagend. We presenteren een rattenmodel van normotherm ex situ donorhartbehoud gevolgd door heterotope abdominale transplantatie. Dit model is relatief goedkoop en kan worden bereikt door een enkele experimentator.

Inleiding

Harttransplantatie blijft de enige levensvatbare therapie voor refractair hartfalen 1,2,3,4. Ondanks een gestage toename van het aantal patiënten dat een harttransplantatie nodig heeft, is er geen evenredige toename van de beschikbaarheid van donororganen waargenomen5. Om dit probleem aan te pakken, zijn nieuwe benaderingen voor het behoud van donorharten ontwikkeld met als doel de uitdagingen te verbeteren en de beschikbaarheid van donoren te vergroten 6,7,8,9.

Normothermische ex situ hartperfusie (NESHP) met behulp van orgaanzorgsysteem (OCS) machines is naar voren gekomen als een klinische interventie 1,3. Deze techniek wordt beschouwd als een geschikt alternatief voor de conventionele statische koude opslag (SCS) methode 2,9. NESHP vermindert effectief de duur van koude ischemie, vermindert de metabole vraag en vergemakkelijkt een optimale voedingsvoorziening en oxygenatie tijdens het transport van donororganen10,11. Ondanks het duidelijke potentieel van deze methode om het behoud van donororganen te verbeteren, zijn de klinische toepassing en het verdere onderzoek ervan beperkt door hoge kosten. Daarom zijn preklinische diermodellen van NESHP cruciaal voor het identificeren van de belangrijkste technische uitdagingen in verband met deze techniek12,13. Varkens en ratten zijn de voorkeursdiermodellen voor preklinische studies vanwege hun ischemische tolerantie9. Hoewel het varkensmodel ideaal is voor fundamenteel en translationeel onderzoek, wordt het beperkt door de hoge kosten en de intensieve arbeid die nodig is voor zorg en onderhoud. Daarentegen zijn rattenmodellen minder duur en gemakkelijker te hanteren14.

In deze studie introduceren we een vereenvoudigd rattenmodel van NESHP, gevolgd door heterotope harttransplantatie, om de impact van de conserveringstechniek op de transplantaatconditie na implantatie te evalueren. Dit model is eenvoudig, kosteneffectief en kan worden uitgevoerd door een enkele experimentator. Figuur 1 toont de schema's van de procedure.

Protocol

De ethische commissie van het Proefdieronderzoekscentrum van het Chonnam National University Hospital (goedkeuringsnr. CNU IACUC - H - 2022-36) keurde alle dierproeven goed. Mannelijke Sprague-Dawley-ratten (350-450 g), gebruikt in deze studie, kregen zorg in overeenstemming met de richtlijnen voor de verzorging en het gebruik van de proefdieren. De ratten werden gehuisvest in temperatuurgecontroleerde kamers met een licht-donkercyclus van 12 uur, met standaard voedsel en water beschikbaar.

1. Voorbereiding

OPMERKING: Een enkele experimentator kan alle experimentele procedures uitvoeren.

  1. Monteer het Langendorff-apparaat, inclusief de oxygenator, pomp en perfusielijnen, voorafgaand aan de operatie (figuur 2). Vul het perfusiecircuit met 20 ml zoutoplossing en circuleer het totdat het is geprimed met autoloog bloed.
    OPMERKING: Het doel van deze stap is om het extracorporale circuit op te warmen.
  2. Bevestig de cardioplegische lijn aan het circuit via de stopkraan die aan de canule van de aorta is bevestigd en bereid de spuitpomp voor op de laatste cardioplegische infusie.
    OPMERKING: Zorg voor het verwijderen van eventuele luchtbellen uit het perfusiecircuit en de cardioplegische lijn.
  3. Plaats de temperatuursensor in het reservoir waar het donorhart wordt opgeslagen, waarbij de temperatuur van het circuit op 37 °C wordt gehouden.
  4. Chirurgische preparaten
    1. Bereid een aparte set steriele micro-instrumenten en materialen voor voor elke donor- en ontvangende rat.
      1. Bereid de chirurgische set voor de donor voor: paar chirurgische scharen, paar microtangen, scherpe muggentangen, 5-0 zijden hechtingen, wattenstaafjes, 50 ml spuit, perfusielijn voor de cardioplegische oplossing (CPS), spuitpomp, 18 G angiokatheter, één set van 5 Fr. femorale katheters en steriele gaasjes.
      2. Bereid de chirurgische set voor de ontvanger voor: microchirurgische schaar, wondretractor, paar microtangen, muggentangen, vasculaire microklemmen, 1 ml spuit, één 5-0 en 9-0 polypropyleen hechtingen, 5-0 zijden hechtingen, wattenstaafjes en steriele gaasjes.

2. Behoud van het donorhart en bloedafname

  1. Induceer anesthesie bij de donorrat met isofluraan (5%) in de anesthesiekamer en noteer het gewicht van de rat voordat u het op de operatietafel legt.
  2. Plaats de rat in rugligging op de operatietafel en dien continue anesthesie toe door 2% -2,5% isofluraan met 90% zuurstof via een neuskegel af te geven.
  3. Controleer de diepte van de anesthesie door het gebrek aan reactie op de teenknijp en de ademfrequentie te controleren, die tussen 50-60 per minuut moet liggen.
    OPMERKING: Een adequaat niveau van anesthesie is cruciaal om onnodige stress en pijn voor de donorrat te voorkomen.
  4. Breng oogglijmiddel aan en scheer de regio schaambeen naar de clavicula, waar de operatie zal worden uitgevoerd. Reinig het gebied met een scrub op basis van jodium en 70% alcohol.
  5. Katheterisatie
    1. Maak een 7 cm midline abdominale incisie en bilaterale incisies van 3 cm van het xiphoid-proces tot het middensleutelbeen. Verwijder de vacht uit het thoracale gebied.
    2. Gebruik wattenstaafjes om de buikorganen aan de linkerkant van de buik te mobiliseren. Isoleer de abdominale aorta van de retroperitoneale fascia en vetweefsels.
    3. Injecteer 1.000 IE heparine opgelost in 0,3 ml isotone zoutoplossing door de inferieure vena cava (IVC) met behulp van een spuit van 1 ml. Stop eventuele bloedingen uit het naaldgat door voorzichtig samen te drukken met een wattenstaafje.
      OPMERKING: Wees voorzichtig met luchtembolie tijdens de injectie, omdat dit kan leiden tot een hartstilstand.
    4. Breng een 5 Fr. femorale katheter in de abdominale aorta (Abd. A). Zorg ervoor dat de katheterpunt de aortaboog bereikt. Bevestig de locatie van de katheter door de geschatte lengte van het ingebrachte deel van de katheter te beoordelen.
  6. Bloedafname
    1. Verzamel ongeveer 10 ml bloed via de katheter die in de Abd. A wordt ingebracht.
    2. Verdun later het primingbloed met isotone zoutoplossing totdat het totale volume 12 ml bereikt. Voeg 5 mg cefazoline opgelost in 0,3 ml zoutoplossing en insuline (20 IE) toe.
  7. Hartstilstand
    1. Sluit de eerder voorbereide CPS-perfusielijn aan op de buikkatheter en start de CPS-toediening met de spuitpomp met een snelheid van 800 ml/uur.
    2. Open de thoracale holte van het diafragma en snijd de IVC dicht bij het diafragma om ventriculaire uitzetting te voorkomen. Snijd de ribben bilateraal langs de thoracale wervelkolom tot aan de thoracale inlaat. Reflecteer de gemobiliseerde ventrale borstwand superieur met een muggentang.
    3. Verwijder de thymus volledig met behulp van een microtang om de aortaboog te visualiseren. Pas lichte compressie toe als de thymusslagaders bloeden.
  8. Extractie
    1. Na het toedienen van alle CPS, isoleer de aortaboog van de omliggende weefsels. Ontleed voorzichtig net onder de linker subclaviaslagader.
    2. Transect de brachiocephalic en links gemeenschappelijke halsslagaders op een verre positie, waardoor de langere stronken van de aortaboog voor eenvoudige hantering tijdens aorta cannulatie. Transect de hoofdlongslagader (MPA) zo dicht mogelijk bij de bifurcatie. Wees voorzichtig om het linker atriale aanhangsel niet te beschadigen.
    3. Limeer voorzichtig de superieure vena cava (SVC) en IVC met 5-0 zijden hechtingen, waardoor de obstructie van het rechter atrium (RA) en de coronaire sinus wordt voorkomen. Bedek de linkerranden van de thorax met nat gaas, plaats het hart erop en trek de SVC- en IVC-ligaturen voorzichtig in om het hilum bloot te leggen.
    4. Ligaat de long- en azygosaders samen met een 5-0 zijden hechting. Snijd het weefsel dorsaal naar de ligatuur en extraheer het hart. Onderzoek het hart op eventuele verwondingen. Weeg ten slotte het hart vóór de aorta-cannulatie.

3. Perfusie ex situ

  1. Aorta cannulatie en perfusie
    1. Vervang vóór aorta-cannulatie het zoutoplossing-geprimeerde circuit door bloedpriming.
    2. Steek de aortacanule in de aortaboog en zet deze vast met een tijdelijke microklem. Zorg ervoor dat de punt van de canule zich op de brachiocephalische overgang bevindt.
    3. Bevestig de juiste positie van de canule door de aorta voorzichtig vast te pakken met een microtang.
    4. Start de perfusie met een stroomsnelheid van 2-3 ml / min, waardoor perfusaat uit de cannulatieplaats kan lekken om eventuele luchtbellen te verwijderen.
    5. Bewaak de perfusiedruk en temperatuur via de sensor die is aangesloten op het bewakingssysteem.
    6. Masseer het hart zachtjes met de eerste en wijsvinger totdat veneus bloed lekt uit de hoofdlongslagader (MPA).
    7. Zet de aorta vast met een 1-0 zijden ligatuur en verwijder de klem na het controleren van alle instellingen (perfusiecircuit, perfusiedruk, temperatuur).
    8. Zodra de permanente ligatuur is geplaatst, moet u ervoor zorgen dat het hart binnen enkele seconden begint samen te trekken en binnen 60 s een normaal ritme bereikt. Een gemiddelde perfusiedruk van 55-65 mmHg met een coronair debiet van 3-4 ml bij 37 °C duidt op voldoende perfusie.
    9. Verzamel 0,15 ml bloed uit het reservoir en controleer de bloedgasanalyse (BGA) aan het begin van de perfusie en daarna elke 20 minuten. Controleer en registreer de pH, pCO 2, pO2, glucose, hematocriet, kalium en lactaat tijdens perfusie. Dien na 120 minuten perfusie 3 ml Custodiol toe via de spuitpomp met een snelheid van 250 ml / uur om het hart te stoppen.

4. Implantatie

  1. Voorbereiding van de ontvanger
    1. Begin met de bereiding van de ontvanger 30 minuten voor het stoppen van ex situ perfusie.
    2. Verdoof het ontvangende dier met dezelfde methode als vermeld in stap 2.2.
    3. Plaats de rat in rugligging op het verwarmingskussen en steek de temperatuurvoeler in het rectum om de lichaamstemperatuur op 37 °C te houden.
    4. Breng oogglijmiddel aan, scheer de schaamstreek naar het epigastrische gebied en reinig het gebied met een scrub op basis van jodium en 70% alcohol.
  2. Medicijnen
    1. Injecteer 2 ml warme zoutoplossing subcutaan om het vocht te compenseren dat tijdens de operatie verloren is gegaan. Injecteer 200 IE heparine subcutaan.
    2. Dien antibiotische profylaxe toe door 10 mg / kg cefazoline opgelost in 0, 3 ml zoutoplossing subcutaan of intramusculair te injecteren.
    3. Dien pijnbestrijding toe door 20 mg/kg diclofenac subcutaan te injecteren.
  3. Voer de laparotomie van de middellijn uit en plaats een retractor om de buikholte te verbreden. Mobiliseer de buikorganen aan de linkerkant van de ontvanger met wattenstaafjes om ruimte te maken voor de procedure.
  4. Voorkom uitdroging door de buikorganen in te pakken met warm en nat gaas. Spreid af en toe warme zoutoplossing met een spuit van 50 ml tijdens de operatie.
  5. Gebruik een chirurgische microscoop met een 10x vergroting, mobiliseer de twaalfvingerige darm en proximaal jejunum door stompe dissectie met wattenstaafjes om de Abd. A. en IVC bloot te leggen. Bereid het Abd. A en IVC voor anastomose en implanteren systematisch het donorhart, in overeenstemming met figuur 3 of eerder gedocumenteerde methoden15.
    OPMERKING: Scheid de Abd. A. en IVC niet.
    1. Ervan uitgaande dat vasculaire anastomose infrarenaal moet worden geplaatst, bereidt u een voldoende deel van de aorta en IVC voor op klemmen.
    2. Voer een stompe voorbereiding uit met wattenstaafjes of een scherp gekartelde tang om de vetten en fascia rond de bloedvaten te verwijderen.
    3. Plaats 5-0 zijden ligaturen op de mesenteriale takken en zowel de craniale als de caudale zijden van de belangrijkste vaten. Til de buikvaten op en slaag of sleiger de lumbale takken met 5-0 zijden hechtingen. Vergeet niet om de testiculaire slagaders en aderen te sparen en ze niet te klemmen.
    4. Gebruik ligaturen om de bloedvaten op te tillen en plaats de microklemmen op de mesenteriale takken, caudale en craniale zijden van de belangrijkste bloedvaten om de bloedstroom op de anastomoseplaats te stoppen. Schakel het verwarmingskussen uit voordat u de klemmen plaatst, omdat overmatige verwarming ischemie van de ledematen kan verergeren. Zorg ervoor dat u het verwarmingskussen inschakelt na het ontklemmen van de vaten om onderkoeling te voorkomen.
    5. Prik de aorta door met een naald van 27 G en verleng de incisie met een microschaar tot een lengte gelijk aan of iets groter dan de opening van de opgaande aorta van de donor (Asc. A), dat is ongeveer 5 mm.
    6. Maak een longitudinale incisie op de IVC op dezelfde manier als de aortotomie, maar maak deze 3 mm dichter bij de caudale kant in vergelijking met de aorta-incisie.
    7. Start de anastomosen, plaats het donorhart aan de rechterkant van de buik van de ontvanger en bevestig de donor-Asc. A naar de Abd van de ontvanger. Een met één eenvoudige onderbroken steek (9-0 polypropyleen) in de schedelhoek van de lengteincisie.
    8. Beweeg het hart naar de linkerkant van de buik van de ontvanger en voer anastomose uit van de asc van de donor. A met de Abd van de ontvanger. A met behulp van een lopende 9-0 polypropyleen hechtdraad.
    9. Fixeer de donorlongslagader op de IVC met twee onderbroken hechtingen (9-0 polypropyleen) in de caudale en craniale hoeken van de longitudinale incisie.
    10. Voer de eerste helft van de veneuze anastomose uit vanaf de intraluminale kant van het vat en voltooi de tweede helft vanaf de extraluminale kant van het vat. Voordat u de knopen aanspant, spoelt u het veld met zoutoplossing om luchtembolie te voorkomen.
  6. Ontluchten en ontklemmen
    1. Verwijder eerst de mesenteriale aderklem na het voltooien van de anastomose om de rechterkant van het hart te laten vullen met veneus bloed.
    2. Verwijder de lucht in het coronaire circuit en Asc. A. door retrograde coronaire perfusie gedurende enkele seconden toe te passen.
    3. Plaats een stuk gaas aan beide zijden van de vaten en verwijder de caudale klem en de schedelklem.
    4. Breng zachte compressie aan met wattenstaafjes gedurende 1-2 minuten. Nadat u voor voldoende hemostase hebt gezorgd, verwijdert u de wattenstaafjes en wast u de anastomosen met een warme zoutoplossing.
      OPMERKING: Het hart moet beginnen te kloppen binnen de eerste minuut van reperfusie. Als de lichaamstemperatuur van de ontvangende rat lager is dan 35 °C, zal het hartritme normaliseren nadat de temperatuur 36 °C heeft bereikt.
  7. Vervang de buikorganen op een meanderachtige manier en sluit de lagen van de abdominale incisie met behulp van continue 5-0 polypropyleennaden.
  8. Plaats het verdoofde dier na de operatie op een schoon gebied boven een verwarmingskussen totdat de lichaamstemperatuur 37 °C bereikt.
    OPMERKING: Start de postoperatieve onderzoeken niet totdat de lichaamstemperatuur 37 °C bereikt. Handhaaf anesthesie op 2-2,5% isofluraan tot het einde van de experimenten.
  9. Controleer het ECG van het getransplanteerde donorhart gedurende 3 uur. Snijd vervolgens het hart onder diepe anesthesie voor histologische studies.
    OPMERKING: Bevestig de anesthesiediepte via een gebrek aan pedaalreflex voordat u het hart exciteert. De chirurgische ingreep en de ECG-monitoring duren minder dan 6 uur. Diclofenac, perioperatief toegediend (stap 4.2.3.), maakt pijnbestrijding mogelijk voor de gehele duur van deze procedure. Het analgesieregime kan worden aangepast volgens de institutionele richtlijnen voor diergebruik.

Resultaten

Figuur 1 illustreert het experimentele ontwerp dat wordt gebruikt in een model met kleine dieren. Figuur 2 toont het gemodificeerde Langendorff-perfusieapparaat, dat een oxygenator voor kleine dieren bevat. De volgorde van anastomose voor heterotope abdominale implantatie is weergegeven in figuur 3.

Discussie

Onze focus bij het opzetten van dit model was om normotherme menselijke harttransplantatie te repliceren. Niet-uitwerpende modellen zijn de algemeen geprefereerde techniek voor het behoud van het donorhart in een ex situ omgeving16. Hoewel uitwerpmodellen veel voordelen bieden bij het beoordelen van de hartfunctie tijdens ex situ perfusie17, zijn ze niet geschikt voor heterotope transplantatiemodellen. 4. Implantation

  1. Preparation of recipient
    1. Begin the recipient preparation 30 min before the cessation of ex situ perfusion.
    2. Anesthetize the recipient animal using the same method as mentioned in step 2.2.
    3. Place the rat in a supine position on the heating pad and insert the temperature probe into the rectum to maintain the body temperature at 37 °C.
    4. Apply eye lubricant, shave the pubic to the epigastric area, and cleanse the area with an iodine-based scrub and 70% alcohol.
  2. Medications
    1. Inject 2 mL of warm saline subcutaneously to compensate for the fluid lost during the surgery. Inject 200 IU of heparin subcutaneously.
    2. Administer antibiotic prophylaxis by injecting 10 mg/kg cefazolin dissolved in 0.3 mL of saline subcutaneously or intramuscularly.
    3. Administer pain control by injecting 20 mg/kg of diclofenac subcutaneously.
  3. Perform the mid-line laparotomy and insert a retractor to widen the abdominal cavity. Mobilize the abdominal organs to the left side of the recipient using cotton swabs to make space for the procedure.
  4. Prevent dehydration by wrapping the abdominal organs with warm and wet gauze. Intermittingly spread warm saline with a 50 mL syringe during the surgery.
  5. Utilizing a surgical microscope with a 10x magnification, mobilize the duodenum and proximal jejunum by blunt dissection with cotton swabs to expose the Abd. A. and IVC. Prepare the Abd. A and IVC for anastomosis and systematically implant the donor heart, in accordance with Figure 3 or previously documented methods15.
    NOTE: Do not separate the Abd. A. and IVC.
    1. Assuming vascular anastomosis to be placed infrarenal, prepare a sufficient portion of the aorta and IVC for clamping.
    2. Perform blunt preparation using cotton swabs or sharp-serrated forceps to remove the fats and fascia around the vessels.
    3. Place 5-0 silk ligatures to the mesenteric branches and both the cranial and caudal sides of the major vessels. Elevate the abdominal vessels and coagulate or ligate the lumbar branches with 5-0 silk sutures. Remember to spare the testicular arteries and veins and do not clamp them.
    4. Use ligatures to lift the vessels and position the micro-clamps to the mesenteric branches, caudal, and cranial sides of the major vessels to stop the blood flow at the anastomosis site. Be sure to switch off the heating pad before placing the clamps, as excess heating can exacerbate limb ischemia.
    5. Puncture the aorta using a 27 G needle and elongate the incision with micro scissors to a length equal to or slightly larger than the opening of the donor ascending aorta (Asc. A), which is approximately 5 mm.
    6. Make a longitudinal incision on the IVC in the same way as the aortotomy, but make it 3 mm closer to the caudal side compared to the aorta incision.
    7. Starting the anastomoses, placed the donor heart on the right side of the recipient's abdomen and attach the donor Asc. A to the recipient's Abd. A with one simple interrupted stitch (9-0 polypropylene) at the cranial corner of the longitudinal incision.
    8. Move the heart to the left side of the recipient abdomen and perform anastomosis of the donor's Asc. A with the recipient's Abd. A using a running 9-0 polypropylene suture.
    9. Fixate the donor pulmonary artery to the IVC with two interrupted sutures (9-0 polypropylene) at the caudal and cranial corners of the longitudinal incision.
    10. Perform the first half of the venous anastomosis from the intraluminal side of the vessel and complete the second half from the extraluminal side of the vessel. Before tightening the knots, flush the field with saline to prevent air embolism.
  6. De-airing and de-clamping
    1. Remove the mesenteric vein clamp first after completing the anastomosis to allow the right side of the heart to fill with venous blood.
    2. Remove the air in the coronary circuit and Asc. A. by applying retrograde coronary perfusion for several seconds.
    3. Place a piece of gauze on both sides of the vessels and remove the caudal clamp and the cranial clamp.
    4. Apply gentle compression with cotton swabs for 1-2 min. After ensuring adequate hemostasis, remove the swabs and wash the anastomoses with warm saline.
      NOTE: The heart should begin beating within the first minute of reperfusion. If the recipient rat's body temperature is below 35 °C, the heart rhythm will normalize after the temperature reaches 36 °C.
  7. Replace the abdominal organs in a meander-like manner and close the layers of the abdominal incision using continuous 5-0 polypropylene sutures.

  8. After the surgery, place the anesthetized animal on a clean area over a heating pad until the body temperature reaches 37°C. 
    NOTE: Do not initiate the postoperative examinations till the body temperature reaches 37°C. Maintain anesthesia at 2-2.5% isoflurane until the end of the experiments.
  9. Monitor ECG of the transplanted donor heart for 3 h. Then, excise the heart under deep anesthesia for histological studies.
    NOTE: Confirm anesthesia depth via lack of pedal reflex before excising the heart. The surgical procedure and the ECG monitoring take less than 6 h. Diclofenac, administered perioperatively (step 4.2.3.), enables pain management for the entire duration of this procedure. The analgesia regimen can be adjusted per the institutional animal use guidelines.
