Active Metabolites Impact

Active metabolites are products of drug biotransformation that retain or develop pharmacological activity, influencing a medicine’s overall therapeutic and safety profile. Enzymes, particularly hepatic cytochrome P450 systems, convert parent compounds into metabolites that may activate receptors, prolong drug action, or produce distinct effects; renal or hepatic impairment can further alter their accumulation and clearance. In clinical practice, evaluating active metabolites helps explain variable treatment responses, delayed or persistent effects, and adverse reactions. This knowledge supports pharmacokinetic analysis, drug interaction assessment, therapeutic drug monitoring, and individualized dosing, especially when metabolism differs because of genetics, organ function, age, or coadministered medicines.
