Healthcare documentation is the systematic recording of a patient’s health information, clinical assessment, care decisions, and outcomes to support safe, coordinated care. Clinicians create and update records by collecting relevant history, examination findings, diagnoses, medication information, treatment plans, and responses to therapy in structured or narrative formats, often within electronic health records. Accurate documentation communicates patient needs across care teams, supports clinical decision-making and continuity of care, and provides evidence for quality improvement, research, billing, and regulatory compliance. Clear, timely records also help reduce errors by making changes in patient status and care plans visible to authorized healthcare professionals.
