Ventilation Perfusion Mismatch

Ventilation-perfusion mismatch is an imbalance between air reaching the alveoli and blood flowing through the pulmonary capillaries, a major cause of impaired gas exchange and hypoxemia. It occurs when regions of the lung receive ventilation without adequate perfusion, as in pulmonary embolism, or perfusion without sufficient ventilation, as in airway obstruction; these patterns are described as high or low ventilation-perfusion ratios. In clinical practice, recognizing this mismatch helps explain abnormal oxygen levels, interpret arterial blood gases and pulmonary function findings, and guide diagnostic evaluation and treatment of disorders such as chronic obstructive pulmonary disease, pneumonia, and vascular disease.
