Core Needle Biopsy

Core needle biopsy is a minimally invasive diagnostic procedure that removes a small cylinder of tissue for microscopic examination, helping clinicians evaluate abnormal masses or lesions without surgical excision. Guided by imaging or physical examination, a hollow, spring-loaded needle passes through the skin and captures tissue cores, which are preserved and analyzed by a pathologist for cellular architecture, malignancy, inflammation, or other disease-related changes. In medicine, the procedure supports diagnosis and treatment planning for conditions affecting the breast, liver, prostate, lymph nodes, and other organs. Compared with open biopsy, it generally requires a smaller incision and shorter recovery, while providing more tissue detail than fine-needle aspiration.

Practicing of fine needle aspiration biopsies (FNAB) by trainees is relatively challenging, due to the lack of an easily available, appropriate lesion. Preparation of an AV phantom lesion for practicing the FNAB procedure and mastering proficiency is relatively easy.

Substernal thyroid lesions are common, and need to be differentiated from malignancy. Obtaining percutaneous fine needle biopsy is not possible due to its retrosternal location. This article proposes a protocol for biopsy of substernal thyroid lesions using Endobronchial Ultrasound-guided Transbronchial Needle Aspiration (EBUS-TBNA).
