Splenectomy

Splenectomy is the surgical removal of the spleen, an organ that filters blood, recycles aging red blood cells, and contributes to immune defense. Surgeons remove the spleen through an open or laparoscopic approach by controlling its blood supply and separating it from surrounding tissues, sometimes evaluating for accessory splenic tissue. The procedure may treat traumatic splenic injury, splenic enlargement, selected blood disorders, or certain tumors when other therapies are insufficient. Because splenectomy reduces clearance of particular bacteria and alters platelet regulation, long-term care may include vaccination, infection-prevention planning, and monitoring for thrombocytosis or other postoperative complications.

2026

This protocol describes a robot-assisted partial splenectomy for a benign splenic tumor. The robotic approach optimizes spleen preservation by enhancing surgical precision, minimizing blood loss, and improving postoperative outcomes. Key surgical steps include meticulous splenic hilum dissection, selective vascular ligation, intraoperative ultrasonography, and controlled parenchymal transection.

With the rapid advancement of laparoscopic techniques, the minimally invasive benefits of laparoscopic splenectomy combined with pericardial devascularization have become increasingly evident. In this context, we present a protocol for performing laparoscopic splenectomy alongside pericardial devascularization to treat hypersplenism and esophageal variceal hemorrhage resulting from portal hypertension.

An Ex Vivo Technique to Generate Tumor-Specific Chimeric Antigen Receptor T Cells

2025

This video demonstrates a technique for the ex vivo generation of tumor antigen-specific chimeric antigen receptor (CAR) T cells. Retroviral vectors carrying transgenic RNA are mixed with spleen cells in a medium supplemented with interleukin-2 to promote T-cell survival and proliferation. The mixture is then transferred to a plate coated with recombinant human fibronectin fragments and centrifuged to facilitate the fusion of the virus with the cell, allowing the RNA to integrate into the host...
