Capsulotomy

Capsulotomy is a neurosurgical procedure that creates small, precisely targeted lesions in the internal capsule, a white-matter pathway linking frontal brain regions with deeper structures; in psychology and psychiatry, it is studied as a treatment for otherwise disabling, treatment-resistant obsessive-compulsive disorder (OCD). The procedure interrupts selected frontostriatal circuits, reducing the transmission of signals involved in persistent thoughts, urges, and repetitive behaviors. Because it permanently alters brain tissue, capsulotomy is reserved for carefully evaluated cases and examined alongside symptom reduction, cognitive and emotional outcomes, and ethical considerations, making it relevant to research on brain-behavior relationships and severe mental illness.
