Ons onderzoek gaat over de toepassing van fluorescentiebeeldvorming bij complexe laparoscopische cholecystectomie. Het belangrijkste doel van ons onderzoek is om de optimale timing en dosering van preoperatieve indocyaninegroeninjectie te achterhalen. De meest recente ontwikkeling in complexe laparoscopische cholecystectomie gaat over de betere visualisatie van galwegen, zoals de fluorescentiebeeldvorming waar we momenteel aan werken.Ons onderzoek heeft de waarde van indocyaninegroen bij complexe laparoscopische cholecystectomie bevestigd en zal nuttig zijn voor een betere intraoperatieve visualisatie van galwegen. Om te beginnen, toedien 0,25 milligram indocyaninegroen aan de patiënt via perifere venepunctie 45 minuten voor de operatie. Stel het DPMI-fluorescentiesysteem in, een dual-kanaals, beeldgestuurd apparaat, om te werken in wit licht en nabij-infrarode spectrums.Plaats nu de patiënt in rugligging op de operatietafel. Na het induceren van algemene anesthesie volgens institutioneel goedgekeurde protocollen, voltooi alle veiligheidsprocedures, inclusief het dragen van een kap, steriele handschoenen en steriele scrub. Creëer een steriele werkruimte en voer trocarinvoer uit met een vier-poorts methode.Schakel het DPMI-fluorescentiesysteem over naar de fluorescentiemodus om de galwegen te lokaliseren. Gebruik de fluorescentienavigatie om voorzichtig de cystische ductus en cystische arterie te disseceren en isoleren met een coagulatiehaak om het kritieke veiligheidsbeeld te bereiken, ook bekend als Calot's driehoek. Gebruik Hem-O-lock clips om de cystische ductus te beveiligen nadat de galwegen zijn geïdentificeerd en onderscheiden.Na het doorknippen van de cystische ductus, knijp de cystische arterie door. Vervolgens dissecer de galblaas van zijn fossa met geschikte instrumenten. Gebruik de witlichtmodus om te inspecteren op eventuele bloedingen.Schakel over naar de fluorescentiemodus om de afwezigheid van gallek te bevestigen na het voltooien van de cholecystectomie. Verwijder de galblaas uit de buikholte met behulp van een extractiezak. Plaats een drainagebuis bij de galblaasfossa.Sluit de buikholte met 4-0 polyglactinnaden voor de interne lagen en hecht de huid met 3-0 niet-resorbeerbare hechtingen. Bewaak de herstel van de patiënt na de operatie nauwgezet. Voer tests uit om de gezondheid te beoordelen en eventuele afwijkingen op te sporen.Fluorescentie-gestuurde laparoscopische cholecystectomie werd met succes uitgevoerd bij drie patiënten, zonder omzetting naar open chirurgie en zonder galwegverwonding waargenomen tijdens de procedures. Er werd geen postoperatieve gallek, infectie of buikpijn waargenomen bij de patiënten.
