Hormonal Regulation

Trawienie obejmuje układ neuroendokrynny, w którym mózg monitoruje i uwalnia hormony trawienne w różnych momentach procesu.

W żołądku, w fazie żołądkowej, gdy wykrywane są białka, gruczoły uwalniają dwa hormony, gastrynę i histaminę, które stymulują produkcję kwasu solnego i zwiększają soki żołądkowe, które rozkładają pokarm na treść pokarmową.

Pojawienie się treści pokarmowej w dwunastnicy powoduje uwolnienie kilku hormonów, które wyłączają proces trawienia w żołądku i zaczynają stymulować ruch treści pokarmowej przez resztę układu.

Na przykład peptyd hamujący żołądek (GIP) spowalnia burczenie w żołądku. Sekretyna i cholecystokinina lub CCK indukują następnie zamknięcie zwieracza między żołądkiem a dwunastnicą, ograniczając uwalnianie treści pokarmowej.

Sekretyna hamuje również produkcję soku żołądkowego i stymuluje uwalnianie wodorowęglanów z trzustki w celu neutralizacji kwasowości treści pokarmowej, a CCK stymuluje enzymy trawienne z trzustki i żółć z pęcherzyka żółciowego.

Wreszcie, peptyd YY jest hormonem uwalnianym przez jelito kręte, koniec jelita cienkiego i okrężnicę. Spowalnia ruch treści pokarmowej do jelita grubego, dzięki czemu woda i elektrolity mogą być łatwo wchłaniane. Ogranicza to objętość treści pokarmowej w dwunastnicy, wpływając na tempo trawienia.

Gdy treść pokarmowa znajdzie się w dwunastnicy, sekretyna powoduje uwolnienie wodorowęglanów z trzustki. Zmniejsza to kwasowość treści pokarmowej, chroniąc wrażliwą wyściółkę dwunastnicy i tworząc optymalne środowisko, w którym mogą funkcjonować enzymy trawienne. Enzymy trawienne i żółć są uwalniane z trzustki i pęcherzyka żółciowego po stymulacji przez CCK, umożliwiając kontynuację trawienia w jelicie cienkim.

Na końcu jelita cienkiego, w jelicie krętym, uwalniany jest inny hormon: peptyd YY (PYY), gdy treść pokarmowa przechodzi do jelita grubego. Hormon ten jest uwalniany z czasem, osiągając szczyt około 1-2 godzin po jedzeniu. Jego funkcją jest spowolnienie przejścia treści pokarmowej do jelita grubego, aby umożliwić maksymalne wchłanianie wody i elektrolitów. Służy również jako sygnał sytości dla mózgu, wskazując organizmowi, aby przestał jeść. PYY może odgrywać rolę w otyłości. Niski poziom PYY zaobserwowano u osób otyłych w porównaniu z osobami nieotyłymi. Badany jest syntetyczny analog PYY jako możliwy lek na otyłość.

Digestive hormones control when the stomach makes acid and when food moves into the small intestine. They work with the neuroendocrine system, which links nerve signals and hormone release during digestion.

In the stomach, proteins are detected by sensory neurons in the enteric nervous system. This stimulates the pyloric gland to release gastrin. Gastrin then helps trigger histamine release, and together they start hydrochloric acid production. The acid helps break food down into chyme. As the stomach becomes more acidic, a negative feedback loop stops both hormones.

After chyme enters the duodenum, several hormones are released. Gastric inhibitory peptide, or GIP, slows stomach churning. Secretin lowers gastric juice production and, together with cholecystokinin, or CCK, causes the pyloric sphincter to close between the stomach and duodenum. This limits how much chyme enters the duodenum and helps pace digestion.

Secretin also prompts the pancreas to release bicarbonate. Bicarbonate reduces the acidity of the chyme. This protects the lining of the duodenum and creates the right conditions for digestive enzymes to work. CCK also stimulates the pancreas and gallbladder to release digestive enzymes and bile, so digestion can continue through the small intestine.

Near the end of the small intestine, in the ileum, peptide YY, or PYY, is released as chyme moves toward the large intestine. PYY rises over time and peaks about 1 to 2 hours after eating. It slows chyme movement so water and electrolytes can be absorbed. It also sends a satiety signal to the brain, which helps tell the body to stop eating. Low PYY levels have been seen in obese compared with non-obese individuals, and a synthetic PYY analog is being studied as a possible obesity treatment.
