Lipid Digestion

Lipid digestion begins in the mouth, where glands on the tongue release lingual lipase. This enzyme starts breaking large fat molecules, mainly triglycerides, into smaller parts.Next, in the acidic environment of the stomach, gastric lipase breaks down fats. Lingual lipase also becomes more active and continues breaking down fats. Because fats do not mix well with water, they tend to clump together, which slows digestion.When fats enter the duodenum of the small intestine, bile salts surround the fat droplets and break them into many smaller droplets. This forms an emulsion that disperses the fat droplets throughout the watery environment, increasing the surface area available for pancreatic lipase.With the fats spread out, pancreatic lipase breaks triglycerides into free fatty acids and monoglycerides.Finally, these smaller fat molecules are absorbed through the intestinal epithelium for further processing.

Lipids are large molecules that are generally not water-soluble. Since most of the digestive enzymes in the human body are water-based, there are specific steps the body must take to break down lipids and make them available for use.

<h4>Lingual Lipase and Gastric Lipase</h4>

Lingual lipase is an enzyme secreted by the acinar cells of the sublingual gland that aids lipid digestion. Although found in saliva, it plays only a minimal role in breaking down lipids in the mouth. Interestingly, lingual lipase has a pH optimum of 3.5-6.0 and is not activated until chewed food enters the acidic environment of the stomach. Gastric lipase is an acidic lipase that is secreted by the gastric chief cells in the lining of the stomach.

<h4>Acidic Lipases in Adult Humans and Neonates</h4>

Lingual lipase and gastric lipase comprise the two acidic lipases found in the human digestive system. These lipases are active in the stomach but rapidly inactivated by bile acids in the duodenum. Together, gastric lipase and lingual lipase account for 10-30% of lipid hydrolysis that occurs in human adults, with gastric lipase contributing the most. Given the low concentrations of pancreatic lipase and bile salts in the neonatal phase, the acidic lipases are critical for lipid digestion and account for 50% of lipid hydrolysis in neonates.

<h4>Bile</h4>

Bile contains bile salts, lecithin, and cholesterol-derived substances, so it acts as an emulsifier in the duodenum of the small intestine. The emulsification of the fat droplets in the duodenum increases their surface area over a thousand-fold, thus making them more accessible to pancreatic lipases.

Lipases and bile help the body break down lipids, which are large molecules that do not mix well with water. Because most digestive enzymes are water-based, lipids need special steps before the body can use them.

Lingual lipase is an enzyme made by acinar cells in the sublingual gland. It is found in saliva, but it has only a small role in the mouth. Its best pH, or acidity level, is 3.5-6.0, so it becomes active after chewed food reaches the acidic stomach.

Gastric lipase is another acidic lipase. It is secreted by the gastric chief cells in the stomach lining. Lingual lipase and gastric lipase are the two acidic lipases in the human digestive system, and they work in the stomach before bile acids in the duodenum quickly inactivate them.

In adults, these two lipases account for about 10-30% of lipid hydrolysis, or fat breakdown. Gastric lipase contributes the most. In neonates, however, acidic lipases are much more important. Because pancreatic lipase and bile salts are still low, they account for about 50% of lipid hydrolysis.

Bile contains bile salts, lecithin, and cholesterol-derived substances. In the duodenum, the first part of the small intestine, bile acts as an emulsifier. Emulsification breaks large fat droplets into smaller ones and increases their surface area more than a thousand-fold, which helps pancreatic lipases work more effectively.
