Antegrade Modular Approach

The Antegrade Modular Approach is a medical procedural strategy that reaches a target in the forward or proximal-to-distal direction while using interchangeable components to tailor treatment to patient anatomy. In practice, clinicians establish an antegrade access route, then assemble or position modular instruments, implants, or treatment elements in a planned sequence, allowing each stage to be adjusted independently. This approach can simplify complex procedures, support controlled reconstruction, and improve adaptability when anatomy or operative requirements vary. Its modular design also facilitates procedural planning, component replacement, and technique standardization, making it relevant to surgical training and specialized clinical care.

With advancements in laparoscopic techniques, laparoscopic radical antegrade modular pancreatosplenectomy (L-RAMPS) has been widely recognized. However, owing to several technical difficulties in this procedure, the artery-first approach in L-RAMPS still remains uncommon. Here, we developed the dorsal-caudal artery approach for L-RAMPS, which might be safe and beneficial for pancreatic neck tumors.
