Salivary Cortisol Analysis

Salivary cortisol analysis is a noninvasive method for measuring cortisol, a steroid hormone that helps regulate the stress response, metabolism, and circadian rhythms. Because biologically active, unbound cortisol diffuses from blood into saliva, timed saliva samples can be collected without needles and analyzed to estimate changes in circulating free cortisol, often across the day or after a physiological challenge. In medicine, this approach supports assessment of adrenal function, circadian rhythm disorders, and stress-related conditions, while repeated sampling can improve monitoring of hormone patterns and reduce the confounding effects of acute procedural stress.

Cortisol (CORT) accumulates in the growing hair shaft of humans and nonhuman primates. We describe methods for extracting and analyzing hair CORT with high precision and sensitivity. Measurement of hair CORT is particularly well-suited for assessing chronic stress over periods of weeks to months.

This protocol describes a rapid saliva pretreatment method for improving cortisol detection. A handheld filtration device is used to remove mucins and other interfering substances while preserving the target analyte. The pretreated saliva exhibits improved capillary flow, thereby enabling more consistent and reproducible assay performance.

We demonstrate: (1) procedures for collection of salivary samples in cognitive impaired older adults by family caregivers in the home setting, (2) procedures for measuring stress activity via salivary cortisol and alpha amylase, and (3) representative profiles. Protocols that allow researchers to study stress-linked processes advance our understanding of biological sensitivity and susceptibility.

Hair cortisol concentration analysis provides an alternative to traditional measures of cortisol; however, to collect hair samples from African Americans, scientists need to be culturally informed and competent. The purpose of this protocol is to demonstrate a culturally informed technique to collect hair samples for cortisol analysis from African Americans.
