Single Incision Laparoscopy

Single-incision laparoscopy is a minimally invasive surgical technique that uses one skin incision, often at the umbilicus, to access the abdominal or pelvic cavity while limiting disruption of the abdominal wall. Through a single access port, surgeons introduce a camera and specialized instruments, then create pneumoperitoneum with carbon dioxide to expand the operative space and perform dissection, removal, or repair under video guidance. The approach can support procedures such as cholecystectomy, appendectomy, and selected gynecologic or urologic operations, with potential benefits including fewer visible scars and reduced incision-related trauma; however, limited instrument triangulation and crowding can increase technical difficulty, making patient selection and surgeon expertise important.

This article presents a method for measuring the working abdominal volume during laproscopic myomectomy using the surgical grasper, and applies this technique to obtain pilot data on whether deep neuromuscular blockade (NMB) can enable the use of lower insufflation pressure. Reduced insufflation pressure during laproscopic surgery has been shown to reduce post-operative pain, and thus the use of NMB during surgery may enable improved patient outcomes.

Metacarpal Small Incision for Carpal Tunnel Syndrome

Carpal tunnel decompression with transverse carpal ligament transection is a very successful treatment of carpal tunnel syndrome. The standard long-curved incision technique has become the best treatment for median nerve entrapment decompression. Here we introduce the metacarpal small incision carpal tunnel release (MSICTR) in the treatment of carpal tunnel syndrome (CTS).
