Physiology of the Circulatory System

Em humanos, uma medida da função do sistema circulatório pode ser obtida medindo a pressão arterial. O termo pressão arterial descreve a pressão lateral produzida pelo sangue sobre as paredes dos vasos. 
Uma das aplicações mais importantes da medição da pressão arterial é verificar o aumento da pressão arterial, uma condição denominada hipertensão. Um em cada três adultos nos Estados Unidos sofre de hipertensão, e é uma das principais causas de doenças cardiovasculares. O equipamento necessário para uma medição indireta tradicional da pressão arterial inclui um estetoscópio e um esfigmomanômetro. O esfigmomanômetro consiste em um manguito de pressão arterial contendo uma bexiga distensível e um bulbo de borracha com uma válvula ajustável que, quando fechada, auxilia na inflação do balonete e, quando aberta, libera a pressão acumulada. Também consiste em tubos que conectam o manguito ao bulbo e ao manômetro que exibe a pressão do manguito em milímetros de mercúrio. Para registrar a leitura da pressão arterial, o examinador envolve o manguito ao redor da artéria braquial, coloca um estetoscópio sobre essa artéria, infla o manguito acima da pressão sistólica esperada e, em seguida, esvazia-o enquanto ausculta e observa o manômetro simultaneamente. Inicialmente, quando o manguito está totalmente inflado, a artéria é espremida e o fluxo sanguíneo é interrompido. Assim, não há som na ausculta. Após a deflação, a primeira aparição dos sons de Korotkoff significa a pressão sistólica, que é audível devido ao fluxo turbulento na artéria parcialmente comprimida. A deflação adicional causa uma diminuição contínua da pressão do manguito, e os sons de Korotkoff permanecem audíveis por toda parte, até o ponto em que a pressão do manguito está abaixo da pressão arterial mínima. Esta leitura denota a pressão diastólica. A fração sistólica sobre diastólica é registrada como a leitura final da pressão arterial. 
Outro indicador importante da fisiologia humana é a taxa de respiração, o número de respirações por minuto. O ar que respiramos viaja através de nossa traqueia, em nossos pulmões através dos brônquios. Dentro dos pulmões, ele passa pelos bronquíolos para finalmente entrar nos sacos de ar especializados chamados alvéolos. Os alvéolos são circundados por capilares sanguíneos que permitem a difusão do oxigênio inalado em nossa corrente sanguínea e facilitam a excreção de dióxido de carbono, mantendo assim a homeostase do nosso sistema. Para mamíferos como os humanos, manter a homeostase inclui a preservação de uma temperatura corporal relativamente constante, independente da temperatura ambiente circundante. Essa propriedade nos torna endotérmicos. Portanto, nossa taxa metabólica e taxa de respiração permanecem relativamente constantes. Pelo contrário, quase todos os peixes, incluindo peixes dourados, têm uma temperatura corporal interna que quase corresponde à temperatura do ambiente ao seu redor. Tais organismos são chamados de ectotérmicos. Como eles não mantêm ativamente uma temperatura interna constante, sua taxa metabólica e respiratória muda para compensar a variação na temperatura ambiente circundante. <h4>Homeostase</h4>
As condições no ambiente externo de um organismo podem mudar rápida e drasticamente. Para sobreviver, os organismos devem manter um ambiente interno bastante constante, o que envolve a regulação contínua da temperatura, pH e outros fatores. Esse estado equilibrado é conhecido como homeostase, que descreve os processos pelos quais os organismos mantêm suas condições internas ideais. Para manter a homeostase, os organismos desenvolveram estruturas com funções distintas. A fisiologia é o estudo das funções e mecanismos normais dos diferentes sistemas do corpo. Por exemplo, se o ambiente externo ficar mais quente do que a temperatura interna ideal, o organismo ativará processos fisiológicos que impedirão o corpo de aquecer até a temperatura externa. Os humanos e muitos outros animais atingem a homeostase por meio de processos fisiológicos distintos em células especializadas.
<h4>Sistemas de órgãos em humanos</h4>
Um ou mais tipos de células com funções de suporte formam tecidos, que por sua vez compõem órgãos com funções corporais especializadas. Os sistemas de órgãos consistem em dois ou mais órgãos que trabalham juntos para fornecer uma função comum. O sistema fisiológico dos vertebrados compreende 11 sistemas de órgãos principais. Embora todos os sistemas de órgãos estejam interconectados, eles funcionam de forma independente uns dos outros.
O corpo é coberto pelo sistema tegumentar, que consiste em pele, cabelo, unhas, receptores sensoriais e várias glândulas. Regula o crescimento, o metabolismo e a reprodução em conjunto com outros sistemas. O sistema respiratório controla as trocas gasosas para fornecer oxigênio ao corpo, pois remove o dióxido de carbono dos pulmões após a passagem de ar pela cavidade nasal, faringe, laringe, traqueia e brônquios. O sistema digestivo processa e decompõe os alimentos que são ingeridos pela cavidade oral e pelo esôfago e, em seguida, movidos pelo estômago, intestinos delgado e grosso antes da excreção pelo reto e ânus. Os nutrientes são absorvidos nos intestinos delgado e grosso e depois processados pelo fígado. O sistema urinário concentra e elimina os resíduos nitrogenados através do rim, bexiga e uretra. Também livra o corpo do excesso de água. O sistema cardiovascular ou circulatório consiste no coração, vasos sanguíneos e sangue, e fornece oxigênio e nutrientes aos tecidos, enquanto remove dióxido de carbono e resíduos por todo o corpo. O sistema linfático mantém a resposta imune do corpo por meio de glóbulos brancos ou linfócitos (alojados na medula óssea vermelha), timo, vasos linfáticos, ducto torácico, baço e gânglios linfáticos. Finalmente, o sistema reprodutivo prepara as células reprodutivas dos organismos. Nos homens, os testículos e o pênis compreendem o sistema reprodutivo, enquanto nas mulheres o útero, os ovários e a vagina compreendem o sistema reprodutivo.
<h4>Fisiologia do Sistema Circulatório</h4>
A fisiologia de organismos unicelulares e animais multicelulares basais, como esponjas, costuma ser simples. Por exemplo, o tamanho pequeno e a grande proporção superfície/volume dos microrganismos permitem que eles atinjam a regulação por difusão através da membrana celular. Da mesma forma, a água do mar circula pelos poros das esponjas, transportando nutrientes e resíduos de e para suas células. Animais mais complexos desenvolveram sistemas circulatórios para mover o sangue por todo o corpo para transportar nutrientes, resíduos, hormônios e outras moléculas, enquanto os sistemas respiratórios permitem a troca gasosa entre o sistema circulatório e o ambiente externo. 
O sistema circulatório em animais pode ser aberto ou fechado. Os sistemas circulatórios abertos estão tipicamente presentes em muitos invertebrados e consistem em um ou mais corações simples, uma rede de vasos e espaços interconectados que banham diretamente os órgãos internos em um fluido que permite a troca de materiais. Os vertebrados têm sistemas circulatórios fechados, nos quais o sangue é confinado dentro de um sistema de vasos fechados que se ramifica extensivamente nos tecidos para garantir a troca de material. Este sistema de vasos fechados está conectado a um coração com veias que transportam sangue dos tecidos para o coração e artérias que transportam sangue do coração para o resto do corpo. Corações de quatro câmaras, como em humanos, estão associados a duas alças de vasos. Em humanos, o sangue pobre em oxigênio dos órgãos entra no coração através do átrio direito, que se contrai para empurrar o sangue para o ventrículo direito, que por sua vez envia sangue para os pulmões. Após a troca gasosa nos pulmões, o sangue rico em oxigênio retorna ao átrio esquerdo e é posteriormente empurrado para o ventrículo esquerdo. Os sistemas circulatórios fechados permitem a rápida circulação do sangue, o que, por sua vez, permite o transporte rápido e eficiente de substâncias por todo o corpo, bem como uma pressão arterial mais alta do que nos sistemas abertos. A pressão arterial é gerada pela contração dos ventrículos cardíacos à medida que o sangue é forçado a entrar nas artérias. À medida que os ventrículos do coração relaxam, a pressão arterial diminui.
<h4>Tensão arterial</h4>
Em humanos, o funcionamento do sistema circulatório pode ser avaliado medindo a pressão arterial e a frequência cardíaca correspondente em um indivíduo. A pressão arterial é medida em milímetros de mercúrio (mmHg), que é a altura em milímetros para a qual o mercúrio em uma coluna é elevado devido à pressão exercida sobre ele. A frequência cardíaca é medida em batimentos por minuto. Por causa dos movimentos de contração e relaxamento do coração, as leituras da pressão arterial consistem em dois números - sistólica e diastólica. A pressão sistólica é medida durante as contrações dos ventrículos e a pressão diastólica é a pressão mínima nas artérias durante o repouso entre as contrações. Geralmente, pressões sistólicas de 90-120 mmHg e pressões diastólicas de 60-80 mmHg são consideradas saudáveis. Quando se trata de frequência cardíaca, 60-100 batimentos por minuto é considerado saudável para adultos. Os atletas geralmente têm frequências cardíacas mais baixas, pois os exercícios cardiovasculares elevam a frequência cardíaca e condicionam o coração a bombear com mais eficiência, o que eventualmente reduz a frequência cardíaca em repouso.
Níveis elevados de pressão arterial por longos períodos ou hipertensão podem danificar os vasos sanguíneos e têm sido associados a ataques cardíacos e derrames. Os pesquisadores descobriram que os efeitos cardiovasculares da pressão sistólica e diastólica são diferentes, de modo que as taxas de eventos cardiovasculares estão fortemente associadas à pressão sistólica. Portanto, o número de pacientes com hipertensão sistólica que sofrem de eventos cardiovasculares é maior do que o número de pacientes com hipertensão diastólica3. Fatores genéticos, bem como estilo de vida e fatores ambientais podem causar hipertensão e doenças cardiovasculares. Por exemplo, o consumo de grandes quantidades de sal causa retenção de excesso de água no corpo, elevando a pressão arterial e sobrecarregando os vasos sanguíneos. Quaisquer insultos aos vasos sanguíneos os tornam propensos a lesões, o que fornece superfícies para o acúmulo de placas, eventualmente enrijecendo os vasos sanguíneos e reduzindo a eficiência do fluxo sanguíneo.
Medições de pressão arterial
Os esfigmomanômetros são usados para medir a pressão arterial. Eles são compostos por um manguito inflável que é conectado a uma bomba (manual ou automática) e um manômetro. A maneira mais eficiente de usar um esfigmomanômetro é na artéria braquial na parte superior do braço, que está no nível do coração. Os esfigmomanômetros são usados em conjunto com um estetoscópio, que é um dispositivo médico acústico usado para ouvir sons internos por meio de um disco de metal ou ressonador. O estetoscópio é mantido logo abaixo do esfigmomanômetro, logo acima da parte interna do cotovelo do indivíduo, para medir os sons da pressão arterial sistólica e diastólica. O manguito é inflado a 200 mmHg, interrompendo o fluxo sanguíneo beliscando os vasos sanguíneos e é uma quantidade segura de pressão a ser aplicada ao braço. À medida que o manguito esvazia, os vasos sanguíneos começam a se abrir e o sangue pode fluir novamente. A pressão arterial sistólica é representada pelo primeiro ruído ouvido e a pressão diastólica é determinada pelo último ruído ouvido. Esses ruídos são chamados de sons de Korotkoff, que são o som do sangue sendo empurrado à força através dos vasos pelo coração4.
<h4>Sistema respiratório</h4>
O sistema circulatório trabalha em estreita colaboração com o sistema respiratório para fornecer oxigênio aos tecidos enquanto remove o dióxido de carbono. Diferentes organismos desenvolveram estruturas respiratórias distintas para as trocas gasosas. Por exemplo, muitos animais aquáticos trocam gases através de brânquias. Os movimentos branquiais são facilmente observados e podem ser usados para calcular a taxa de respiração de organismos aquáticos contando o número de vezes que o organismo move sua cobertura branquial ou opérculo. A taxa de respiração pode mudar com a temperatura porque as moléculas de oxigênio são transportadas em taxas diferentes, dependendo de quão quente ou frio é o ambiente. Em um ambiente aquático, a quantidade de oxigênio dissolvido disponível na água diminui com o aumento da temperatura. A diminuição do oxigênio tem efeitos sobre a taxa de respiração dos organismos aquáticos, dada sua capacidade de difundir oxigênio por todo o corpo. Por outro lado, os animais terrestres possuem estruturas respiratórias internas, como os pulmões. Em humanos, a respiração envolve a inalação contraindo o diafragma para puxar o ar. Quando o diafragma relaxa, o ar é liberado passivamente dos pulmões.
O tabagismo é a principal causa de câncer de pulmão, responsável por 80-90% das mortes por câncer de pulmão. Todos os anos, mais de 120.000 americanos morrem de câncer de pulmão associado ao tabagismo e representam uma grande parcela das mortes evitáveis5. Em conjunto, o estilo de vida contribui para a saúde dos sistemas circulatório e respiratório e uma quantidade significativa de mortes pode ser evitada por mudanças no estilo de vida.

circulatory system: blood flow and pressure

The circulatory system moves blood, oxygen, nutrients, and wastes through the body to support homeostasis. Homeostasis is the stable internal balance that organisms must maintain even when the outside environment changes. Physiology is the study of how body systems work, and the circulatory system is one of the key systems that helps keep internal conditions steady.

Body systems are built from cells, tissues, organs, and organ systems. Humans have 11 major organ systems, and they work together even though each system has its own main job. The integumentary system protects the body and helps control temperature. The skeletal system supports organs and gives muscles a place to attach. The muscular system moves the body and can also move substances through hollow organs. The nervous system controls responses to stimuli. The endocrine system uses hormones to regulate growth, metabolism, and reproduction. The respiratory system exchanges gases in the lungs. The digestive system breaks down food and absorbs nutrients. The urinary system removes nitrogenous wastes and extra water. The lymphatic system helps immune defense. The reproductive system produces reproductive cells.

In simple organisms, transport can happen by diffusion because of their small size and large surface area to volume ratio. Sponges also rely on water flow through their pores to carry nutrients and wastes. More complex animals need circulatory systems to move materials farther and faster. These systems transport nutrients, waste products, hormones, and other molecules, while the respiratory system supports gas exchange with the outside environment.

Animal circulatory systems can be open or closed. Open circulatory systems are common in many invertebrates. They use one or more simple hearts, a vessel network, and open spaces that bathe organs in fluid for exchange. Vertebrates have closed circulatory systems, where blood stays inside vessels that branch through tissues. In this system, veins carry blood toward the heart, and arteries carry blood away from the heart.

Humans have a four-chambered heart with two circulation loops. Oxygen-poor blood from the body enters the right atrium, moves to the right ventricle, and is sent to the lungs. After gas exchange, oxygen-rich blood returns to the left atrium and then moves to the left ventricle. The left ventricle has a thicker muscle wall because it must pump blood to the rest of the body. Closed circulatory systems move blood quickly and efficiently and produce higher blood pressure than open systems.

Blood pressure and heart rate are used to measure how well the circulatory system is working. Blood pressure is measured in millimeters of mercury, or mmHg. Heart rate is measured in beats per minute. Blood pressure has two numbers. Systolic pressure is the pressure during ventricular contraction. Diastolic pressure is the lowest arterial pressure between contractions. Healthy adult values are usually about 90-120 mmHg for systolic pressure and 60-80 mmHg for diastolic pressure. A healthy adult heart rate is usually 60-100 beats per minute, and athletes often have lower resting heart rates because exercise makes the heart pump more efficiently.

High blood pressure, or hypertension, can damage blood vessels and is linked to heart attack and stroke. The transcript notes that systolic pressure is strongly associated with cardiovascular events, and systolic hypertension is more closely linked to those events than diastolic hypertension. Hypertension can result from genetic, lifestyle, and environmental factors. For example, eating a lot of salt can cause the body to retain extra water. That raises blood pressure and strains blood vessels. Injured vessel walls can also encourage plaque buildup, which can stiffen vessels and reduce blood flow.

Blood pressure is often measured with a sphygmomanometer and a stethoscope. A sphygmomanometer uses an inflatable cuff, a pump, and a pressure gauge. It is usually placed on the brachial artery in the upper arm, level with the heart. The cuff is inflated to 200 mmHg, which briefly stops blood flow and is safe for the arm. As the cuff deflates, blood starts flowing again. The first sound heard is the systolic pressure, and the last sound heard is the diastolic pressure. These sounds are called Korotkoff sounds, and they come from blood being forced through the vessels.

The circulatory system also works closely with the respiratory system to supply oxygen and remove carbon dioxide. Many aquatic animals use gills for gas exchange, and their respiration rate can be estimated by counting operculum movements. Respiration rate can change with temperature because oxygen moves differently in warm or cool environments. In water, dissolved oxygen decreases as temperature rises, which affects aquatic organisms that rely on diffusion. Terrestrial animals use internal structures such as lungs. In humans, inhalation happens when the diaphragm contracts and pulls air in, and exhalation happens when the diaphragm relaxes. Smoking is the leading cause of lung cancer and accounts for 80-90% of lung cancer deaths. The transcript also notes that more than 120,000 Americans die each year from smoking-associated lung cancer, showing how lifestyle choices can affect both circulatory and respiratory health.
