Water Bottle Silhouette

The water bottle silhouette is a chest radiographic sign in which the cardiac outline appears enlarged, smooth, and globular, resembling a water-filled bottle. It most often results from pericardial effusion, as fluid accumulates within the pericardial sac and expands the outer contour around the heart, while the internal chambers may not be proportionally enlarged. This finding can raise clinical suspicion for significant fluid accumulation but is not specific and may be absent in rapidly developing effusions. Clinicians use it alongside symptoms, physical examination, electrocardiography, and echocardiography to assess severity, identify hemodynamic risk, and guide urgent management.
