Donor Heart Preservation

Donor heart preservation is the set of methods used to maintain a donated heart’s structure and function between organ recovery and transplantation. Conventional static cold storage slows cellular metabolism by cooling the heart in a preservation solution, while newer ex vivo perfusion systems circulate oxygenated, nutrient-containing fluid to support the beating or metabolically active organ under controlled conditions. These approaches limit ischemic injury, preserve myocardial viability, and help transplant teams assess organ function before implantation. Improved preservation can extend transport distances, increase the usable donor pool, and support safer heart transplantation, particularly when donor and recipient hospitals are far apart or the heart has experienced circulatory stress.

This paper describes the protocol for use in static cold storage of rodent donor hearts; however, the protocol can also be used for donor hearts obtained after donation after circulatory death and brain death.
