Alpha One Pi Index

The Alpha One Pi Index is a clinical measure used to interpret alpha-1 proteinase inhibitor, also called alpha-1 antitrypsin, status and identify possible deficiency. Alpha-1 antitrypsin normally protects lung tissue by inhibiting neutrophil elastase; inherited SERPINA1 variants can reduce its concentration or impair its function, increasing susceptibility to emphysema and liver disease. Interpreting the index alongside serum protein levels and Pi phenotype or genotype helps clinicians assess inherited risk, guide confirmatory testing, and support evaluation of unexplained respiratory or hepatic disease. In medicine, this information can improve recognition of alpha-1 antitrypsin deficiency and inform monitoring and management decisions.
