Pathogenic bacteria control virulence gene expression by sensing environmental and host signals. This regulation helps them turn virulence factors on only when conditions favor infection and survival. It also helps them adjust to changing pressures during the infection process.

Several regulatory strategies are used in bacteria. Two-component systems use a membrane sensor kinase and a cytoplasmic response regulator to pass along signals by phosphorylation. Quorum sensing lets bacteria detect cell density through signaling molecules, which helps coordinate biofilm formation, virulence, and motility. Posttranscriptional regulation uses RNA-binding proteins and small RNAs to change mRNA stability and translation. Posttranslational modification changes a protein after it is made, such as by phosphorylation or cleavage. Antigenic and phase variation help bacteria evade immune attack by changing surface structures through genetic recombination or slipped-strand mispairing.

The BvgAS system in Bordetella pertussis is a well-studied two-component regulatory system. BvgS is the membrane-bound sensor kinase, and BvgA is the cytoplasmic response regulator. Under host-like conditions, such as 37 °C, BvgS autophosphorylates and transfers the phosphate to BvgA. The activated form, BvgA~P, turns on the Bvg+ phase.

In the Bvg+ phase, Bordetella pertussis expresses genes for adhesins, toxins, and secretion systems. These virulence factors help the bacterium colonize the host and affect the immune response. When low temperature or certain chemical signals inhibit BvgS kinase activity, BvgA is no longer phosphorylated. This shifts the cells into the Bvg− phase, where virulence genes are repressed and genes that support environmental survival are activated.

A third state, the Bvgi phase, allows only limited BvgA phosphorylation. In this phase, the bacterium expresses only a subset of virulence genes, mainly adhesins, while toxins stay suppressed. This intermediate state may help the bacterium adapt during transitions between the environment and the host.
