Preload Afterload Control

Preload and afterload control describes how the cardiovascular system regulates the forces that determine cardiac filling and the resistance the heart must overcome to eject blood. Preload reflects ventricular end-diastolic volume and myocardial fiber stretch, while afterload primarily reflects arterial pressure and vascular resistance; changes in venous return, blood volume, contractility, and vessel diameter alter these variables and influence stroke volume through the Frank-Starling mechanism. Understanding preload and afterload helps explain how the heart maintains cardiac output during exercise, changes in posture, and blood loss, and provides a foundation for interpreting cardiovascular disease, hemodynamic measurements, and therapeutic interventions.

The Frank-Starling-Sarnoff curve is clinically important and describes the relationship between cardiac preload and output. This report illustrates a novel method of simultaneous jugular venous and carotid arterial Doppler velocimetry as transient surrogates of cardiac preload and output, respectively; this approach is enabled by wireless, wearable Doppler ultrasound.
