Renal Compensation

Renal compensation is the kidney’s adjustment of blood chemistry to help restore acid–base balance when another system, usually the lungs, causes a disturbance. In response to respiratory acidosis or alkalosis, renal tubules alter hydrogen ion secretion, bicarbonate reabsorption or production, and ammonium excretion; these changes develop over hours to days and complement rapid respiratory control. This physiological process helps maintain blood pH within a narrow range and supports stable cellular function. Understanding renal compensation is important in biology and medicine because it clarifies how the body responds to chronic breathing disorders, metabolic challenges, and clinical acid–base abnormalities.
