D3 Lymphadenectomy

D3 lymphadenectomy is an extended surgical procedure that removes regional lymph nodes at the pericolic, intermediate, and central levels along the main blood vessels supplying a tumor, most commonly in colorectal cancer. The operation follows the tumor’s lymphatic drainage pathway and dissects lymphatic tissue toward the origin of the relevant feeding vessels, often as part of an oncologic bowel resection. By retrieving more central nodes, D3 lymphadenectomy can improve pathological staging and help identify patients who may benefit from adjuvant treatment. Its potential effect on cancer control must be balanced against operative complexity and risks to surrounding vessels, nerves, and organs.
