Post-discharge Monitoring

Post-discharge monitoring is the structured observation of a patient’s health after leaving a hospital or other care setting, helping clinicians identify recovery problems and support safe transitions to outpatient care. It combines scheduled follow-up, review of symptoms and medications, and, when appropriate, remote collection of vital signs or other patient-reported data; concerning changes trigger clinical assessment or escalation. In clinical practice, this process can reveal complications, treatment barriers, and worsening disease before they require emergency care. Effective monitoring improves continuity, reinforces self-management, and informs care plans for patients recovering from surgery, acute illness, or chronic disease exacerbation.
