Fertility Preservation Surgery

Fertility preservation surgery comprises procedures that protect or retain reproductive potential when disease, treatment, or anatomical conditions could impair future fertility. Depending on the clinical situation, surgeons may conserve the uterus or part of an ovary, reposition ovaries away from a planned radiation field, or remove reproductive disease while minimizing damage to healthy tissue. These approaches can support later conception or the use of assisted reproductive technologies, although outcomes depend on age, ovarian or testicular reserve, diagnosis, and treatment effects. In medicine, fertility preservation surgery is particularly relevant to cancer care, endometriosis management, and other conditions requiring reproductive surgery.

We present details of laboratory procedures for drug-free in vitro activation (IVA) of ovarian follicles for patients with severe ovarian dysfunction. This method could increase the number of retrievable oocytes per ovarian hyperstimulation and benefit fertility preservation for those patients.

The present protocol describes a method for assessing ovarian reserve in patients under 25 years old who require fertility preservation through ovarian tissue cryopreservation. This method involves: (1) histological assessment of ovarian reserve in cortical samples, (2) comparison to a reference dataset, and (3) calculation of Z-scores.
