Gastric Pouch Creation

Gastric pouch creation is a surgical technique that forms a small reservoir from the upper portion of the stomach, most commonly during Roux-en-Y gastric bypass, to support treatment of severe obesity. Surgeons use stapling or suturing to separate the pouch from the remaining stomach, limiting its volume and directing ingested food into a connected segment of the small intestine. This altered anatomy promotes earlier satiety and reduces nutrient absorption, while also influencing digestive hormone signaling. The procedure is used in bariatric medicine to achieve substantial, sustained weight loss and improve obesity-related conditions such as type 2 diabetes and hypertension.

An optimized technique for the microsurgical creation of arterial bifurcation aneurysms mimicking bifurcation human cerebral aneurysms is described. A venous pouch is sutured into an artificially created true bifurcation of both common carotid arteries. Facilitated microsurgical techniques and aggressive postoperative anticoagulation and analgesia lead to minimized morbidity rates and high aneurysm patency rates.

After excising a segment of the right CCA, the pouch is treated with elastase, then sutured to the left CCA to restore blood flow.
