Microsurgical Limb Transplantation

Microsurgical limb transplantation is a reconstructive procedure that replaces a missing hand, arm, foot, or leg with a donor limb, aiming to restore both form and function. Surgeons use microsurgical techniques to align and connect bones, tendons, muscles, nerves, arteries, and veins, while immunosuppressive therapy helps prevent rejection of the transplanted tissue. The procedure can improve movement, sensation, and independence for selected patients with severe limb loss, although it requires extensive rehabilitation and lifelong monitoring. As a form of vascularized composite allotransplantation, it also advances research in tissue integration, immune management, and functional recovery.

Vascularized Composite Allotransplantations (VCA) have become a clinical reality. However, broad clinical application of VCA is limited by chronic multi-drug immunosuppression. The authors present a reliable and reproducible large animal model to translate novel immunomodulatory strategies that can minimize or potentially eliminate the need of immunosuppression in VCA.
