Microsurgical Liver Procurement

Microsurgical liver procurement is the precise surgical recovery of a donor liver using magnification and fine instruments to preserve the organ and its critical vascular and biliary structures. Surgeons carefully dissect and divide the hepatic artery, portal vein, hepatic veins, and bile duct while minimizing tissue trauma, then flush and preserve the graft under controlled conditions for transplantation. This technique is particularly important when vessels are small, anatomy is complex, or a graft requires reconstruction, including selected living-donor procedures. By protecting blood flow and bile drainage, microsurgical procurement supports graft quality, reduces technical complications, and improves the safety and effectiveness of liver transplantation.

This article contains a step by step protocol to procure and prepare the liver graft for machine perfusion, prepare the perfusion fluid, prime the perfusion machine and perform oxygenated normothermic machine perfusion of the liver graft.
