Porcine Liver Transplantation

Porcine liver transplantation is a form of xenotransplantation in which a pig liver, or liver-derived tissue, supports or replaces liver function in a human recipient, offering a potential response to donor-organ shortages and acute liver failure. The approach must address species-specific differences in immune recognition, blood-group compatibility, and coagulation, often through donor-animal genetic modification, immunosuppressive therapy, and careful control of surgical and postoperative conditions. Research evaluates whether porcine livers can provide metabolic detoxification, protein synthesis, and other essential functions while limiting rejection, inflammation, and infection. These studies inform the development of temporary bridging therapies and future clinical transplantation strategies.

The porcine model of liver normothermic machine perfusion (NMP), described here, can be successfully used to study NMP as a preservation strategy, a tool for viability assessment, and a platform for organ repair. It holds a high translational value, however it is technically challenging and labor-intensive.

Preservation of Porcine Donation after Circulatory Death (DCD) Liver by Perfusion and Orthotopic Liver Transplantation

This protocol outlines the methodology of establishing a porcine model utilizing variable temperature-controlled Machine Perfusion (MP) for the preservation of the donor liver, followed by orthotopic liver transplantation (OLTx). It aims to promote the success rate of OLTx using donor donation after circulatory death (DCD) liver and establish a stable model.

Study of the animal organ physiology can be achieved by performing an experimental ex vivo perfusion system. The addition of a porcine kidney, as a homeostatic organ to our previously developed ex vivo liver perfusion model can be a principal step to achieve a better physiological environment.
