Post-discharge Nutrition

Post-discharge nutrition is the planned provision of food, nutrients, and dietary guidance after a patient leaves the hospital, supporting recovery during the transition to home or another care setting. It works by matching energy, protein, fluid, and micronutrient intake to the patient’s illness, treatment effects, functional status, and ability to eat, while addressing symptoms such as nausea, swallowing difficulty, or poor appetite. Clinicians use nutrition screening, individualized meal plans, oral supplements, and follow-up monitoring to reduce nutritional decline and support wound healing, immune function, strength, and medication tolerance. Effective management can improve recovery, promote independence, and reduce avoidable readmissions.
