Thoracic Endovascular Repair

Thoracic endovascular repair (TEVAR) is a catheter-based procedure that treats disease in the thoracic aorta, often providing an alternative to open surgery. During TEVAR, a collapsible stent graft advances through an artery, typically in the groin, to the affected segment, where it expands and lines the aortic wall. The graft redirects blood flow through a reinforced channel, excluding an aneurysm from circulation or sealing an entry tear in a dissection; this can reduce rupture risk and support treatment of selected traumatic aortic injuries. TEVAR requires imaging to guide deployment and monitor graft position, aortic remodeling, and potential complications during follow-up, making it important in modern vascular and emergency medicine.

A New Murine Model of Endovascular Aortic Aneurysm Repair

Histological and biochemical modifications after aneurysm endograft exclusion are still unclear. We describe a new model of endograft implantation on a murine aneurysm. Through thrombus analysis with and without persistant circulating blood stream interface, and new endovascular biomaterials evaluation, this model will help to better understand endovascular aneurysm exclusion pathobiology.

Simulation of complex, high-risk procedures is critical to the education of medical trainees. A protocol for simulator-based endovascular neurosurgery training in a controlled academic environment is described. The protocol includes stepwise guidelines for trainees of varying levels, with a discussion of the advantages and limitations of this model.

Surgery is the gold standard for accessible arteriovenious malformations (AVMs), and pre-operative embolization can simplify this procedure. We describe our approach for staged endovascular embolization and open resection of AVMs, and provide a representative clinical example highlighting the advantages of a comprehensively trained neurovascular surgeon leading a multi-disciplinary clinical team.
