True Lumen Compression

True lumen compression is the narrowing of a blood vessel’s original channel when an adjacent false lumen or expanding intramural collection displaces or presses against it, a clinically important mechanism of vascular malperfusion. In aortic dissection, blood enters through an intimal tear and separates the vessel wall layers; pressure within the false lumen can flatten the true lumen, while a mobile dissection flap may worsen obstruction during the cardiac cycle. Computed tomography angiography, ultrasound, and intravascular imaging help assess the compression and its effects on branch vessels. Recognizing this process guides urgent management aimed at restoring blood flow and preventing ischemic injury to vital organs.
