Veno-arterial Ecmo

Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is a form of temporary life support that assists both the heart and lungs when conventional treatment cannot maintain adequate circulation or oxygenation. Blood is drained from a large vein, pumped through an artificial membrane oxygenator that adds oxygen and removes carbon dioxide, then returned to an artery, thereby supporting systemic perfusion while reducing the workload on the failing heart. VA-ECMO is used in selected patients with refractory cardiogenic shock, cardiac arrest, or severe cardiac and respiratory failure, and may serve as a bridge to recovery, transplantation, or longer-term mechanical support. Its use requires careful management of anticoagulation, bleeding, thrombosis, and organ perfusion.

This protocol outlines the use of point-of-care ultrasound (POCUS) to monitor patients on peripheral veno-arterial extracorporeal membrane oxygenation (V-A ECMO) without left ventricular venting. It evaluates left ventricular distension, intracardiac or aortic root thrombus, and identifies useful cardiac POCUS parameters for weaning from V-A ECMO.

Here we present a protocol describing the technique of veno-venous extracorporeal membrane oxygenation (ECMO) in a non-intubated, spontaneously breathing mouse. This murine model of ECMO can be effectively implemented in experimental studies of acute and end-stage lung diseases.

Surgical correction of ALCAPA is highly recommended, regardless of age or the degree of intercoronary collateralization. This protocol presents a technique for the direct re-implantation of adult-type ALCAPA into the aorta to re-establish the dual-coronary perfusion. Whenever feasible, direct re-implantation is preferred to other surgical correction techniques.

Porcine Liver Transplantation Without Veno-Venous Bypass As an Extended Criteria Donor Model

In this protocol, a model of porcine orthotopic liver transplantation after static cold storage of donor organs for 20 h without the use of a veno-venous bypass during engraftment is described. The approach uses a simplified surgical technique with minimization of the anhepatic phase and sophisticated volume and vasopressor management.
