Imatinib Gleevec

Imatinib, marketed as Gleevec, is a targeted anticancer drug that inhibits specific tyrosine kinases driving abnormal cell growth. It binds the ATP-binding site of kinases such as BCR-ABL, c-KIT, and platelet-derived growth factor receptors, preventing phosphorylation signals required for proliferation and survival. This mechanism makes imatinib a foundational treatment for Philadelphia chromosome-positive chronic myeloid leukemia and certain acute lymphoblastic leukemias, as well as c-KIT-positive gastrointestinal stromal tumors. Its development established molecularly targeted therapy as a major pharmacological strategy and provides a model for matching treatment to the genetic and signaling features of cancer cells.

Robotic Duodenal Sleeve Resection for Gastrointestinal Stromal Tumor with Rare Exon 8 KIT Mutation Following Neoadjuvant Imatinib

This video case report demonstrates the unique management of a duodenal gastrointestinal stromal tumor with a rare, understudied, exon 8 KIT mutation. Neoadjuvant therapy resulted in a 30% reduction in tumor size, making the patient a candidate for a robotic-assisted duodenal sleeve resection instead of the more morbid pancreaticoduodenectomy.
