Sodyum Hidroksit ve Sodyum Deoksikolat Kullanılarak Deselülarize İnsan Periferik Sinir Greftlerinin Hazırlanması

Bu protokol, etkili hücresel uzaklaştırma ve doğal hücre dışı matris mimarisinin korunmasıyla deselülarize insan periferik sinir greftleri üretmek için sodyum hidroksit işlemini peristaltik pompa güdümlü bir solüsyon değişim sistemiyle birleştiren standartlaştırılmış bir alkali destekli deselülarizasyon yöntemini açıklamaktadır.

Özet

Periferik sinir yaralanmaları genellikle aksonal rejenerasyon için yapısal rehberlik sağlayan greft materyaller gerektirir. Otolog sinir grefti kullanımı klinik altın standart olmaya devam etse de, kullanımı donör bölge morbiditesi ve sınırlı doku mevcudiyeti nedeniyle kısıtlıdır. Bu kısıtlamalar, sinir rejenerasyonu için alternatif bir biyomateryal olarak deselülerize insan periferik sinir greftlerine olan ilginin artmasına yol açmıştır. Ancak, yaygın olarak kullanılan deterjan bazlı deselülerizasyon protokolleri, hücre dışı matris (ECM) mimarisini bozabilir ve greft kalitesini tehlikeye atabilecek sitotoksik kalıntılar bırakabilir. Bu makalede, alkali destekli bir deselülerizasyon yaklaşımı kullanılarak deselülerize insan periferik sinir greftlerinin hazırlanması için standartlaştırılmış bir protokol açıklanmaktadır. Prosedür, etkili hücresel yıkımı teşvik etmek için sodyum hidroksit (NaOH) işlemi ile başlar ve ardından, doğal ECM organizasyonunu korurken geri kalan hücresel ve immünojenik bileşenleri uzaklaştıran hafif bir sodyum deoksikolat (SDC) adımı ile devam eder. Bu strateji, doğal ECM mimarisinin korunmasını desteklerken deterjan maruziyetini sınırlandırır. Giriş

Periferik sinir yaralanmaları ciddi fonksiyonel bozukluklara yol açabilir ve rejeneratif tıp ile rekonstrüktif cerrahide önemli bir klinik zorluk olmaya devam etmektedir. Segmental sinir defektlerinin rejenerasyonu, sadece yaralanma boşluğunu köprülemekle kalmayıp aynı zamanda aksonal rejenerasyonu ve reinnervasyonu desteklemek için yapısal rehberlik ve biyokimyasal ipuçları sağlayan greft materyalleri gerektirir. Otolog sinir greftleme, doğal hücre dışı matris (ECM) mimarisini koruduğu ve sinir rejenerasyonu için destekleyici bir mikroçevre sağladığı için yaygın olarak klinik altın standart olarak kabul edilmektedir1,2. Ancak, klinik kullanımı donör doku mevcudiyetinin kısıtlılığı, ek cerrahi prosedürlere duyulan ihtiyaç ve donör bölge morbiditesi ile sınırlıdır3,4. Bu sınırlamalar, özellikle immunojenik hücresel bileşenleri minimize ederken doğal ECM mimarisini koruyan deselülerize periferik sinir greftleri başta olmak üzere alternatif greft materyallerinin geliştirilmesini tetiklemiştir5,6,7,8.

Deselülerize sinir greftlerinin performansı, deselülarizasyon sürecine büyük ölçüde bağlıdır. Geleneksel yöntemler tipik olarak, lipid membranlarını çözerek hücresel bileşenleri uzaklaştıran Triton X-100 ve sodyum deoksikolat (SDC) gibi deterjan bazlı reaktiflere dayanır9,10,11. Ancak, bu deterjanlara uzun süre maruz kalmak ECM mikro yapısını bozabilir ve hücresel canlılığı ile rejeneratif sonuçları olumsuz etkileyen kalıntı sitotoksik maddeler bırakabilir12,13. Sonuç olarak, ECM bütünlüğünü korurken hücrelerin verimli bir şekilde uzaklaştırılmasını sağlayan alternatif deselülarizasyon stratejileri aktif olarak araştırılmaktadır14. Sodyum hidroksit (NaOH) kullanılarak yapılan alkali deselülarizasyon, ekstrem pH koşullarının hücresel bileşenleri, nükleik asitleri ve lipidleri etkili bir şekilde degrade edebilmesi nedeniyle potansiyel bir alternatif yaklaşım olarak ortaya çıkmıştır15,16,17. Buna rağmen, güçlü alkali koşullara kontrolsüz maruziyet ECM bütünlüğünü tehlikeye atabilir; bu durum, hassas bir şekilde kontrol edilen işlem koşullarının önemini vurgulamaktadır16,18.

Burada, peristaltik pompa güdümlü sürekli bir solüsyon değişim sistemi içeren, insan periferik sinir greftleri için standardize edilmiş alkali destekli bir deselülarizasyon protokolü sunulmaktadır. Geleneksel statik daldırma yöntemleriyle karşılaştırıldığında, bu dinamik süreç sürekli solüsyon değişimine olanak tanıyarak hücresel döküntülerin ve kalıntı işlem ajanlarının uzaklaştırılmasını kolaylaştırır. Bu makale; donör sinir hazırlığı, kontrollü alkali destekli deselülarizasyon ve peristaltik pompa güdümlü bir sistem kullanılarak gerçekleştirilen sürekli solüsyon değişimi dahil olmak üzere tüm iş akışının adım adım görsel bir gösterimini sunmaktadır. Elde edilen greftler, etkili deselülarizasyonu ve ECM ultrastrüktürünün korunduğunu doğrulamak için histolojik analiz, kalıntı DNA miktarının belirlenmesi ve taramalı elektron mikroskobu (SEM) kullanılarak değerlendirilmiştir. Kritik teknik adımların net bir şekilde görselleştirilmesini sağlayan bu makale, deselülarize sinir greftlerinin hazırlanması ve gelecekteki işlem ölçeklendirmeleri için net bir şekilde tanımlanmış bir çerçeve sunmaktadır.

Protokol

Donated human peripheral nerves were obtained from a U.S. tissue bank under an exclusive agreement with L&C BIO and in compliance with the regulations of the American Association of Tissue Banks (AATB) and the U.S. Food and Drug Administration (FDA). Donor information, including age and sex, medical histories, and serological test results, was provided with the tissues and reviewed and approved by a certified medical director. Upon receipt, all tissues underwent incoming inspection, including verification of donor screening, serological testing, and testing for relevant pathogens; only tissues that met the predefined acceptance criteria were used in this study. All procedures involving human specimens were conducted in accordance with the principles of the Declaration of Helsinki. As no human subjects were directly recruited or involved in this study, a separate Institutional Review Board (IRB) approval number was not issued.

1. Prepare the solutions (Day 1)

  1. Prepare 0.25 N sodium hydroxide (NaOH) solution by dissolving 10 g of NaOH in 1 L of distilled water under magnetic stirring.
  2. Prepare 4% sodium deoxycholate (SDC) solution by dissolving 40 g of SDC in 1 L of distilled water under magnetic stirring.
  3. Transfer each prepared solution into sterile glass bottles.
    CAUTION: Handle sodium deoxycholate (SDC) inside a chemical fume hood. Wear appropriate personal protective equipment, including gloves, a laboratory coat, and eye protection, to prevent inhalation and skin contact.

2. Prepare the peripheral nerve segments for decellularization

  1. Disinfect the outer surface of the packaged raw peripheral nerve and open the package aseptically.
  2. Rinse the peripheral nerve several times with sterile distilled water.
  3. Remove residual adipose tissue, connective tissue, unnecessary branches, and damaged nerve tissue using sterile scissors and forceps. Rinse the peripheral nerve again with sterile distilled water.
  4. Cut the peripheral nerve into segments measuring 2–5 cm in length. Classify the prepared segments into three length groups: 2–3 cm, >3–4 cm, and >4–5 cm.
    NOTE: Process each length group separately because different IPA, NaOH, and SDC treatment durations are applied according to nerve segment length, as summarized in Table 1.

3. Set up the peristaltic pump system

  1. Attach chemically resistant silicone tubing of sufficient length to both ends of the chromatography column.
  2. Connect one end of the tubing to the peristaltic pump.
  3. Connect the free ends of the tubing to a 1 L glass bottle containing isopropyl alcohol (IPA) using pump tubing to establish a closed-loop circulation system.
    NOTE: Secure all tubing connections firmly, as pressure and vibration from the peristaltic pump may cause the tubing to disconnect from the column or bottle.

4. Delipidize the peripheral nerve

NOTE: Unless otherwise indicated, perform all solution-exchange steps at room temperature (20–25 °C) using a peristaltic pump set to 90–186 mL/min, depending on the tubing configuration. Maintain the selected flow rate constant throughout each processing run.

  1. Open one end of the chromatography column and partially fill the column with the IPA solution to ensure that the nerve tissue remains immersed immediately after insertion.
  2. Using sterile forceps, gently grasp the outer tissue at one end of a single prepared peripheral nerve segment. Avoid directly compressing or grasping the main body of the nerve. Carefully lower the nerve segment into the solution within the column.
  3. Using the forceps, gently straighten the nerve segment and position it longitudinally, parallel to the direction of fluid flow. Ensure that the segment is not folded, coiled, or pressed against the inner wall of the column or the outlet filter.
  4. Close the column and position it at a slight upward incline toward the outlet, with the outlet placed slightly higher than the inlet. This orientation helps prevent the nerve segment from settling against either end filter, particularly the outlet filter.
    NOTE: Process one peripheral nerve segment per column per procedure. No additional physical fixation device is required. The natural buoyancy of the nerve tissue and the relatively gentle fluid flow help maintain the segment in a suspended and extended position. Inspect the tissue position during processing and gently reposition it with sterile forceps if it adheres to the column wall, filter, or itself.
  5. Connect the IPA solution to the peristaltic pump system. Perform continuous solution exchange through the column for 1.5 h for nerve segments measuring 2–3 cm, 2 h for segments measuring >3–4 cm, or 2.5 h for segments measuring >4–5 cm.
  6. Drain the solution from the column.
  7. Add 40 mL of sterile distilled water to the column. Gently invert or shake the column five times to wash the peripheral nerve.
  8. Repeat steps 4.6 and 4.7 for a total of five washes.
  9. Connect a reservoir containing 5 L of sterile distilled water to the peristaltic pump system. Perform continuous solution exchange through the column overnight.
    PAUSE POINT: After completing the overnight wash, the procedure may be paused. Stop the peristaltic pump and store the peripheral nerve fully submerged in sterile distilled water at 4 °C for up to 24 h before proceeding to step 5.1.
    NOTE: Minimize air bubbles inside the column to promote efficient solution exchange. Adjust the peripheral nerve segment as needed to keep it suspended in the solution and prevent it from adhering to the inner wall of the column or filter.

5. Perform the first decellularization (Day 2)

  1. Connect the 0.25 N NaOH solution to the peristaltic pump system. Perform continuous solution exchange through the column for 6 h for nerve segments measuring 2–3 cm, 8 h for segments measuring >3–4 cm, or 10 h for segments measuring >4–5 cm.
  2. Drain the solution from the column.
  3. Wash the peripheral nerve as described in steps 4.6–4.8.
  4. Connect a reservoir containing 5 L of sterile distilled water to the peristaltic pump system. Perform continuous solution exchange through the column overnight.
    PAUSE POINT: After completing the overnight wash, the procedure may be paused. Stop the peristaltic pump and store the peripheral nerve fully submerged in sterile distilled water at 4 °C for up to 24 h before proceeding to step 6.1.

6. Perform the second decellularization (Day 3)

  1. Connect the 4% (w/v) SDC solution to the peristaltic pump system. Perform continuous solution exchange through the column for 6 h for nerve segments measuring 2–3 cm, 8 h for segments measuring >3–4 cm, or 10 h for segments measuring >4–5 cm.
  2. Drain the solution from the column.
  3. Wash the peripheral nerve as described in steps 4.6–4.8.
  4. Connect a reservoir containing 5 L of sterile distilled water to the peristaltic pump system. Perform continuous solution exchange through the column overnight.
    PAUSE POINT: After completing the overnight wash, the procedure may be paused. Stop the peristaltic pump and store the peripheral nerve fully submerged in sterile distilled water at 4 °C for up to 24 h before proceeding to step 7.1.

7. Sterilize the decellularized peripheral nerve (Day 4)

  1. Remove a small volume of distilled water from the column. Carefully retrieve the decellularized peripheral nerve while keeping it suspended in the remaining liquid.
  2. Package the peripheral nerve together with sterile distilled water.
  3. Sterilize the packaged peripheral nerve by gamma irradiation at a dose of 25 kGy.
    NOTE: Do not completely drain the distilled water from the column. Keep the nerve segment suspended in a sufficient volume of the remaining liquid to prevent it from adhering to the inner wall of the column and to facilitate gentle handling without allowing the liquid to overflow.

Sonuçlar

An overview of the preparation process for decellularized human peripheral nerve grafts is shown in Figure 1. The workflow included trimming of the raw peripheral nerve, delipidization with IPA, sequential chemical decellularization with NaOH and SDC, followed by extensive washing and sterilization (Figure 1).

The peristaltic pump-driven solution-exchange system used for delipidization, decellularization, and washing is shown in Figure 2A. Trimming and segmenting of the nerve were illustrated in Figure 2B–D. After completion of the process, the peripheral nerve maintained its overall morphology without visible structural disruption (Figure 2E).

Histological evaluation of the decellularization is shown in Figure 3. Hematoxylin and eosin (H&E) staining of the native peripheral nerve revealed abundant nuclei, whereas nuclei were largely absent in the decellularized samples (Figure 3A). Quantification of residual DNA, expressed as ng DNA/mg of dry tissue, demonstrated a substantial reduction in DNA content following decellularization (Figure 3B).

Ultrastructural analysis using scanning electron microscopy (SEM) is presented in Figure 4. The fibrous organization of the extracellular matrix (ECM) remained clearly visible after decellularization, indicating preservation of the native ECM architecture (Figure 4).

Assessment of residual detergent and cytocompatibility is shown in Figure 5. Residual SDC levels were minimal following the washing steps (Figure 5A). Cytotoxicity testing showed no significant cytotoxic response in cultured cells exposed to the decellularized peripheral nerve grafts (Figure 5B).

Tartışma

Bu çalışma, periferik sinir rejenerasyonu için deselülerize insan periferik sinir greftleri üretmek amacıyla standartlaştırılmış, alkali destekli bir deselülarizasyon protokolü sunmaktadır. Protokol; donör sinir ön işlemi, kontrollü kimyasal deselülarizasyon ve peristaltik pompa güdümlü bir solüsyon değişim sistemini net bir şekilde tanımlanmış bir iş akışında birleştirerek, yerel ECM mimarisini korurken hücresel ve immünojenik bileşenlerin verimli bir şekilde uzaklaştırılmasını sağlar.

Bu yöntemin temel özelliği, alkali işlemin azaltılmış deterjan maruziyeti ile entegre edilmesidir. Sondell (%3 Triton X-100 ve %4 SDC) ve Hudson yöntemi (sulfobetaine-10, sulfobetaine-16 ve Triton X-200) gibi geleneksel deterjan bazlı protokoller, etkili bir deselülarizasyon sağlamak için uzun süreli ve çok aşamalı deterjan tedavilerine dayanmaktadır9,10. Bu yaklaşımlar hücresel bileşenleri etkili bir şekilde uzaklaştırsa da, uzun süreli deterjan maruziyeti ECM bütünlüğünü bozabilir ve kalıntı deterjan kaynaklı sitotoksisiteye katkıda bulunabilir19,20,21. Buna karşılık, mevcut protokol, SDC maruziyetini sınırlarken hücresel uzaklaştırmayı artırmak için NaOH içermekte, böylece toplam deterjan yükünü azaltmakta ve doğal ECM mimarisinin korunmasını desteklemektedir.

Bu protokolün bir diğer kritik yönü, peristaltik pompa destekli solüsyon değişim sistemidir. Doku üzerinde iç basınç oluşturabilen ve yapısal bozulma riski taşıyan21 perfüzyon tabanlı yaklaşımların aksine, bu sistem bir kromatografi kolonu içerisinde sürekli ve kontrollü solüsyon değişimine olanak tanır. Bu yaklaşımın etkinliği, çok sayıda analitik sonuçla desteklenmiştir. Histolojik analiz ve rezidüel DNA kantifikasyonu, hücresel bileşenlerin verimli bir şekilde uzaklaştırıldığını doğrulamış; SEM analizi ise yerli ECM mimarisinin korunduğunu göstermiştir. Ayrıca, düşük rezidüel SDC seviyeleri ve olumlu sitouyumluluk sonuçları, elde edilen greftin biyouyumluluğunu desteklemiştir. Toplu olarak bu bulgular, protokolün etkili deselülarizasyon ile yapısal koruma arasında bir denge kurduğunu göstermektedir.

Prosedürel değişkenliği minimize etmek ve kontrollü işlem koşullarını sürdürmek için birkaç teknik adım kritik öneme sahiptir. İlk olarak, budama, transfer ve yıkama sırasında sinir segmentinin dikkatli bir şekilde taşınması esastır. Periferik sinir dokusu mekanik olarak kırılgandır; aşırı kompresyon, katlanma veya gerilme, dahili yapıyı bozabilir ve ECM korumasını riske atabilir19,20. Bu nedenle, dokular prosedür boyunca nemli tutulmalı ve nazikçe taşınmalıdır. İkinci olarak, kolon içindeki tekdüze solüsyon değişimi, işlem performansının temel bir belirleyicisidir. Kolon içindeki hava kabarcıkları minimize edilmeli ve doku, kolon duvarına veya filtreye yapışmak yerine solüsyon içinde tamamen daldırılmış ve askıda kalmalıdır. Doku yapışması, düzensiz solüsyon değişimine yol açarak yıkama verimliliğini düşürebilir ve işlem reaktiflerine tutarsız maruziyetten kaynaklanan sonuçlar doğurabilir21,22. Üçüncü olarak, peristaltik pompa güdümlü sistem prosedür boyunca stabil çalışmalıdır. Delipidizasyon, deselülarizasyon ve yıkama için sürekli sirkülasyon gereklidir; bu nedenle, pompa kaynaklı basınç veya titreşim nedeniyle ayrılmaları önlemek için tüm tüp bağlantıları güvenli bir şekilde sabitlenmelidir. Numune geri kazanımı sırasında, kolonun tamamen boşaltılmasından kaçınılmalıdır. Yeterli sıvı hacminin korunması, doku yapışmasını önlemeye yardımcı olur ve toplama sırasında nazik taşımayı kolaylaştırır.

2–3 cm uzunluğundaki sinir segmentleri sırasıyla 1,5, 6 ve 6 saat boyunca IPA, NaOH ve SDC ile işlenmiştir. >3–4 cm uzunluğundaki segmentler sırasıyla 2, 8 ve 8 saat; >4–5 cm uzunluğundaki segmentler ise sırasıyla 2,5, 10 ve 10 saat boyunca işlenmiştir. Böylece işlem süreleri, IPA delipidizasyonu için 1,5 ile 2,5 saat arasında, her bir deselülarizasyon adımı için ise 6 ile 10 saat arasında değişmiştir. Reaktif konsantrasyonları, çözelti hacimleri ve yıkama prosedürleri dahil olmak üzere diğer tüm işlem parametreleri, uzunluk grupları boyunca tutarlı şekilde sürdürülmüştür. Borulama konfigürasyonuna bağlı olarak akış hızı, belirtilen aralıkta ayarlanmış ve her bir işlem süreci boyunca sabit tutulmuştur. Uzunluğa özgü işlem koşulları Tablo 1'de özetlenmiştir. Sinir çapı, kalınlığı, fasiküler yapı veya hücresel ve matriks kompozisyonuna dayalı ayarlamalar sistematik olarak değerlendirilmemiş olup daha fazla araştırma gerektirmektedir23,24. Ölçek büyütme uygulamaları için çok kanallı bir peristaltik pompa sistemi verimliliği artırabilir; ancak homojen bir işlem sağlamak için kanallar arasındaki tutarlı akış koşulları dikkatle doğrulanmalıdır.

Daha fazla optimizasyon için çeşitli hususlar geçerliliğini korumaktadır. Bu protokol genel deterjan maruziyetini önemli ölçimde azaltsa da, SDC halen yer almaktadır ve gelecekteki iyileştirmeler, deselülarizasyon verimliliğini ve ECM korumasını sürdürürken deterjan kullanımını minimize etmeye veya deterjanı değiştirmeye odaklanabilir25,26. Ayrıca, periferik sinir dokusu; fasiküler yapı, çap, kalınlık ve hücresel ile matriks bileşimindeki varyasyonlar nedeniyle doğası gereği standartlaştırılması zor bir dokudur23,24. Büyük ölçekli üretim sırasında, bu farklılıklar, doku yükleme ve akış koşullarındaki varyasyonlarla birlikte lotlar arası değişkenliğe katkıda bulunabilir. Bununla birlikte, mevcut çalışmada donörler arası ve lotlar arası tekrarlanabilirlik sistematik olarak değerlendirilmemiştir. Bu nedenle, uygun doku kabul kriterlerini belirlemek, süreçleri standartlaştırmak ve lot tekrarlanabilirliğini sağlamak için birden fazla donörden alınan dokular ve bağımsız üretim lotları kullanılarak yapılacak daha ileri doğrulamalar gerekli olacaktır.

Ek bir kısıtlama ise greft uzunluğuyla ilgilidir. Rejeneratif performans, greft uzunluğu arttıkça azalır ve bu uzunluğa bağlı kısıtlama, aselüler sinir allogreftlerinde otogreftlere göre daha belirgin olabilir3,27. Aselüler sinir allogreftleri yerleşik canlı Schwann hücrelerinden yoksun olduğundan, aksonal rejenerasyonu desteklemek için konakçı Schwann hücrelerinin göçüne ve proliferasyonuna bağımlıdırlar27. Saheb-Al-Zamani ve ark., uzun aselüler sinir allogreftlerinde görülen sınırlı rejenerasyonun, artan Schwann hücresi senesansı ile ilişkili olduğunu bildirmişlerdir27.

Buna rağmen, önceki bir in vivo çalışma, NaOH tabanlı deselülarize insan sinir greftlerinin 15 mm'lik tavşan siyatik sinir defektinde otogreftlerle karşılaştırılabilir düzeyde fonksiyonel ve yapısal iyileşmeyi desteklediğini göstermiştir28. Contreras ve arkadaşları ayrıca, koyunlarda 70 mm'lik bir peroneal sinir defekti boyunca deselülarize bir sinir allogrefti kullanarak aksonal rejenerasyon ve fonksiyonel iyileşme sağladıklarını kanıtlamışlardır29. Bu çalışmalar, deselülarize sinir greftlerinin rejeneratif potansiyelini desteklemektedir; ancak mevcut protokol kullanılarak hazırlanan greftlerin klinik olarak anlamlı greft uzunlukları boyunca daha fazla değerlendirilmesi gerekmektedir. Otogreftler ayrıca donör sinir mevcudiyeti ile sınırlıdır ve ek bir toplama prosedürü gerektirir, bu da ikinci bir operasyon sahasına ve donör bölge morbiditesine yol açar4. Bu nedenle, yeterli otolog doku mevcut olmadığında veya donör sinir toplama işlemi pratik olmadığında, aselüler sinir allogreftleri klinik olarak anlamlı bir alternatif sağlayabilir.

Özetle, bu protokol, deselülarize periferik sinir greftlerinin hazırlanması için pratik ve uyarlanabilir bir çerçeve sunmaktadır. Azaltılmış deterjan maruziyeti, kontrollü solüsyon değişimi ve süreç standardizasyonuna verilen önem, hem araştırma uygulamalarını hem de periferik sinir rejenerasyonundaki potansiyel translasyonel kullanımı desteklemektedir. Gelecekteki çalışmalar; deterjansız yaklaşımların daha fazla geliştirilmesine, sistem ölçeklenebilirliğine ve rejeneratif performansın kapsamlı in vivo değerlendirmesine odaklanmalıdır.
