Electronic Health Record

Electronic health records (EHRs) are digital systems that collect, store, and organize a patient’s longitudinal health information, supporting coordinated and informed clinical care. They replace or supplement paper charts by enabling authorized users to document encounters, retrieve medical histories, and share information through structured data fields and interoperable health information exchange. In clinical practice, EHRs help clinicians review diagnoses, medications, allergies, test results, and treatment plans, while decision-support tools can flag potential risks and support preventive care. Aggregated, de-identified EHR data also contribute to quality improvement, population health monitoring, and clinical research, although data quality, privacy, security, and interoperability remain important considerations.
