Isovalerylcarnitine C5

Isovalerylcarnitine C5 is an acylcarnitine metabolite used as a biochemical marker of disrupted leucine breakdown, particularly in isovaleric acidemia. When isovaleryl-CoA accumulates because isovaleryl-CoA dehydrogenase is deficient, carnitine conjugation produces isovalerylcarnitine, which can be detected in blood by tandem mass spectrometry. Measuring C5 in dried blood spots supports newborn screening and helps identify patients at risk of toxic organic acid accumulation, metabolic acidosis, and infection-triggered decompensation. Interpretation requires attention to clinical context because pivaloylcarnitine from some medications can produce a similar C5 signal.
