Graft Perfusion

Graft perfusion is the delivery of blood or a preservation solution through transplanted tissue or an organ to maintain cellular viability and function before or after implantation. During machine perfusion, a pump circulates oxygenated or chilled fluid through the graft under controlled pressure and flow, allowing metabolism, vascular resistance, and tissue condition to be assessed. This approach supports organ preservation, viability testing, and selection for transplantation, particularly when conventional cold storage may be insufficient. In medicine, graft perfusion can improve assessment of marginal organs, guide preservation decisions, and provide a platform for delivering treatments before transplantation.

Normothermic Ex Vivo Kidney Perfusion for the Preservation of Kidney Grafts prior to Transplantation

The severe organ shortage has resulted in increased use of marginal kidney grafts for transplantation. This has triggered interest in alternative storage methods, since marginal grafts especially tolerate cold storage poorly. The technique of normothermic ex vivo kidney perfusion (NEVKP) represents a novel preservation method for kidney grafts prior to transplantation.

Marginal grafts, such as fatty livers, grafts from older donors, or livers retrieved after cardiac death (DCD) tolerate conventional, cold static storage only poorly. We developed a novel model of subnormothermic ex vivo liver perfusion for preservation, assessment, and repair of marginal liver grafts prior to transplantation.

This article demonstrates the feasibility of achieving longer perfusion times (4 h) of murine cardiac grafts without function loss by employing lower (30-35 mmHg) than physiological (60-80 mmHg) perfusion pressures during Langendorff.

Porcine Normothermic Isolated Liver Perfusion

The porcine model of liver normothermic machine perfusion (NMP), described here, can be successfully used to study NMP as a preservation strategy, a tool for viability assessment, and a platform for organ repair. It holds a high translational value, however it is technically challenging and labor-intensive.

Here we present a protocol describing oxygenated ex situ machine perfusion of donor liver grafts. This article contains a step by step protocol to procure and prepare the liver graft for machine perfusion, prepare the perfusion fluid, prime the perfusion machine and perform oxygenated normothermic machine perfusion of the liver graft.
