Spleen-preserving Pancreatectomy

Spleen-preserving pancreatectomy is a surgical procedure that removes a diseased portion of the pancreas while retaining the spleen, an approach intended to treat selected pancreatic lesions without unnecessary splenic loss. During distal pancreatectomy, surgeons separate the pancreatic tissue from adjacent splenic structures and preserve splenic blood flow, either through the splenic artery and vein or, in selected cases, through collateral vessels. The technique is used mainly for benign, borderline, or low-grade tumors and other localized disease when oncologic principles permit spleen preservation. By avoiding splenectomy, it can maintain splenic function and reduce long-term risks associated with asplenia, while demanding careful vascular and pancreatic management.
